The haunting lyrics and melody of "Nothing Else Matters" by Metallica was the perfect accompaniment to the series of clips from season 12, culminating in Dean kneeling over Castiel's dead body after Lucifer dragged Mary into the alternate reality and Sam found Kelly's dead body and followed the blood trail to Jack.

In a smooth transition, the new season's story picked up right there, where it left off in season 12.

Jack was lost, trying to find his "father" Castiel. The sheriff was kind to him, finding him clothes and trying to help him. Asmodeus, the last Prince of Hell, appeared in Hell. His brother Azazel was killed by Dean in "All Hell Breaks Loose, Part 2". His brother Ramiel was killed by Sam in "Stuck in the Middle with You". And, his sister Dagon was killed by Castiel in "The Future".

The last Prince of Hell made quite an entrance, telling the demons that he was there "to rule until such time as Lucifer returns with his son." He continued, telling them that he would be making changes, that the "corporate lackeys of the Crowley era are being purged," and "the grand old days of fire and brimstone are back." Later in the episode, when the Winchesters found out about him, Dean said, "Hopefully, this fourth Prince of Hell is the last Kardashian in the family."

Lucifer was forcing Mary to stay with him in the alternate reality as he searched for a way out. When she told him that she knew he planned to kill her, he responded, "Why would I kill you? Oh! He took on the form of Donatello and told Jack that God wanted him to help rescue a "troop of God's bravest soldiers" who were trapped in Hell. He told Jack that God wanted him to set the Shadim free, and to "be a hero." Jack started using his power to free the creatures, but the Winchesters arrived and stopped him.

Instead, it was another shapeshifter that had been stalking her.

The cosmic entity, looking exactly like Castiel, appeared to Castiel after the angel woke up. He told Cas that they were in "nothing but empty" and that "angels and demons, you all come here when you die." He then said that they all sleep an "endless, peaceful sleep" and let Castiel know that when Castiel woke up, it woke him, and he was very upset about being awake.

Jack showed Mia the video of Kelly talking to him and told her he had never gotten to meet his mother. He begged her to help him. Mia obliged, appearing as Kelly. The statement worked on two levels - first, from his mother; and second, from a monster that was helping people instead of hurting them.

The shifter tied up Dean and Jack and tried to make Mia kill them. Then, he saw Sam heading toward the house and prepared to shoot him when he came through the door. Sam opened the door, and as the shapeshifter shot at him, Jack tapped his powers, throwing the monster back and diverting the bullet from Sam. Sam was then able to draw his gun and shoot the shifter.

Afterward, Dean told Jack he did good, and apologized to Sam, admitting that he needed his brother to keep the faith for both of them, because he didn't believe in anything at that point.

The celestial entity was forced to be awake if Castiel was awake in the empty. He could not stand it, and he needed Castiel to go back to sleep. He tried to coax Castiel to go back to sleep. He tried torturing him by showing him his past mistakes. He tried beating him up. Nothing worked. Castiel told him, "You can prance, and you can preen, and you can scream and yell, and remind me of my failings, but somehow, I'm awake and I will stay awake and I will keep you awake, until we both go insane. I will fight you and fight you and fight you forever, for eternity. . . Release me! Release me!" Finally, the celestial entity released him and sent him back to earth.
