In the last few moments of "Walk on Water", Meredith was working on an unconscious patient while taking care of a catatonic child. She had just finished sewing up the artery in the man's leg when he awoke. While trying to restrain the patient, Meredith tripped and fell backward into the water.

Derek saw the catatonic child in the midst of the crowd and asked her about Meredith. The little girl led Derek through the crowd to the end of the dock where she pointed at the water.
