The flashback seems to materialize through Jack's "dream" while he is sleeping in the hatch bunker, and he is visibly unsettled when he awakens. (He is soon to be more alarmed to see Locke unconscious and Michael forcing him into the armory room at gunpoint.)
In Jack's first flashback, he is seen with his father examining the case of two desperate individuals hoping for a medical miracle. Angelo and Gabriela had come from Italy as a last hope for his largely-progressed spinal tumor, knowing Jack had saved Sarah from the brink of impossibility once before.
Against his father's advice, Jack accepted the case and developed a personal relationship with Gabriela through the course of Angelo's treatment.
Although coming to Jack was requesting a miracle, Gabriela confessed to Jack that she did not believe in miracles (to which Jack expressed doubt as well, despite his previous encounter with Desmond and Sarah's extraordinary results).
As Jack once again became fully committed to "fixing" another patient, his relationship with Sarah was drifting away. Unfortunately, both Jack's marriage and the attempt to work another wonder end in tragedy by the end of the flashbacks (which finally solve the missing link in the story of Jack's marriage followed in previous episodes).

Michael's prior encounters with the cryptic code-entering computer seem to have led him to believe that it is not at all what it seems to be. Likely, the messages received from "Walt" (the last and most crucial of which was not completely visible) may have ignited his urgency to head off alone to search. (Recall that one message asked specifically if Michael was alone.)
Armed with a gun from the vault and Locke's previous shooting lessons, Michael forced Jack into the armory with the unconscious Locke and took off.
Locke awakens and informs Jack that he had bolted the air vent shut to prevent intruders, and expressed concern about the button. Fortunately Sawyer and Kate arrived to free the two from the vault using the new combination-- which Locke dictates as 25, 29, 40. Jack immediately decides to pursue Michael, with Locke and Sawyer joining the "hunting party".

As the search for Michael began, Sawyer questioned Jack's harsh response to Kate's desire to come. Jack was clearly resentful of Sawyer's delirious profession of "I love her" (which he assumed to be Kate). Meanwhile, Locke works to pick up a trail and sense of direction. Locke identifies what seems to be Michael's boot print (according to Mr. Eko, the "Others" do not leave tracks). It is figured that Michael is heading north, while the tail group (and the rafters) had come from the east-- thus, Michael was heading elsewhere, possibly by instruction from the mysterious messages.
A sense of direction is rather important to consider, as it appears the fuselage is located somewhere southward on the island and the raft had been heading north/northeast in the ocean before the survivors were brought back by the current. The speedboat, in contrast, had come from the southwest corner of the radar screen (having presumably come from the direction of the island, after initially traveling towards it.
The island appears to be largely unexplored aside from three main crash sites and various other discovered destinations-- the Nigerian plane, the Black Rock, Danielle's bunker, etc-- were scattered as well. The origin of the "Others" and their relative location remains unknown, but is possibly the direction in which Michael was headed.

On the beach, Sun and Jin notice an urgent conversation between Kate and Hurley. Hurley briefly mentions Michael's escape and the search party pursuing him, then departs to head for the hatch-- a prelude to where Kate may be going, instead of to the doomsday button.
Meanwhile, Jin is eager to catch up with the group to resume the search for Michael once again (whom he now identifies as a close friend). Sun, however, displays the greatest amount of defiance and assertion when she demands that her husband not go. Jin is hesitant, but agrees.
A mutual understanding seems to conclude the following morning as Jin expresses worry for Michael, as well as a dismay at being given orders from his spouse. Sun replies that she did not like receiving orders from Jin for four years, to which he concedes. A new, far more equal status had been obtained in their relationship through the simple exchange (a large step away from Jin's dominant authority upon first arrival to the island).
Later, after arriving to the hatch, Hurley and Charlie are looking at records and Hurley reveals (to Sayid also) that a search party had gone after Michael.

That is, their marriage was based upon gratitude and the intense care for his "miracle patient"-- a care Jack now displayed for another individual. As seen with Boone's case on the island, and now on his hunt for Michael, Jack possesses a fierce defiance to quitting (in all aspects of survival).
Christian had warned Jack to beware of a "line", which Jack knew existed and could not avoid.
Unfortunately Jack crossed the line once, in an effort to comfort Gabriela after his futile attempts to save Angelo were unsuccessful. Ultimately Sarah ended the marriage, bringing Jack to tears with the knowledge of always needing "something to fix".
The analogy of the "line" which mustn't be crossed resurfaces in an apparently much more literal sense on the island, when the bearded stranger (called, "Mr. Friendly") also warns that the crossing of the "line" dividing the survivors from the "Others" would lead from misunderstanding to... something else.
This line seemed to have actually existed, as Sawyer's bullet was apparently repelled as if by a force field.
To Jack's chagrin, this line was not exceeded.

Still recovering from the infection, Sawyer begins to struggle on the hike uphill and asks for a brief rest (which Jack ignores, until Locke pauses also). Michael seemed to be continuing due north in a path-- as Locke explained--of "a man who knows where he is going". Rivalry and difference in opinion resurface in a tense argument between Jack and Locke concerning whether the pursuit of Michael was necessary or not valid at all. Jack's vehement insistence of bringing Michael back exemplify the evolution which had occurred in Jack's personality since becoming stranded on the island, as well as his slightly overbearing (yet increasing) authority.
Gunfire is heard, at least seven shots ringing through the jungle, although only three shells of which are found astray. (The graze marking of a bullet is also spotted by Locke on a tree.)
The searchers are left to wonder if the rest had met their targets, and Locke begins to retreat from the search due to incoming dusk. Jack argues, continuing to lead towards the gunfire by torchlight and accusing Locke of not wanting to find Michael when the trail is lost.

Although Sawyer identifies the mysterious stranger (whom he had earlier named "Bluebeard", and now referred to as "Zeke") as the man who had shot him on the raft, recall that it was not the bearded man who had shot Sawyer. Instead, it was one of the other men (the identical twins, who were then restraining Walt as the boat pulled away) who had pulled the trigger on Sawyer.
Locke is nearly turning back from the search, insisting that Jack will not be able to enact Michael's return. Jack maintains that if the search party returned, it would be upon their shoulders that Michael would never be seen again.
Suddenly a voice emerges from the darkness in the jungle; "You're exactly right, Jack!"
The shadow of a fourth figure was illuminated by the flames, and suggested that they all listen to "Mr. Locke". The man is revealed to be none other than bearded man from the speedboat, suggested by some to bear close resemblance to Mr. Gerald DeGroot. (Recall the co-founder of the Dharma Initiative was seen in the orientation film, along with Karen DeGroot who appears similar to Bluebeard's female bomb-tossing companion.)
Anxious for revenge, Sawyer shoots at "Mr. Friendly" is wounded by his own bullet when the bullet is returned by an apparent force field between the two groups.
Earlier Jack had laid question to Sawyer's motives for joining the hunting party, doubting that Sawyer had come for Michael as much as for revenge on the shooters. It seems apparent that Sawyer is highly vindictive of the "Others" and carries many personal motives, although a sense of indebtedness to Michael is plausible as well. Michael's refusal to leave Sawyer behind (essentially saving his life, although they had been bitter enemies) on the journey with the tail group-- and Sawyer's repentance at have left Michael behind the first time-- may have influenced his determination as well.

In the hatch, Hurley and Charlie are busy entertaining themselves with old record albums and discussions about their women troubles. While Charlie seeks re-acceptance of Claire, Hurley confides in his hope for a chance with newcomer Libby.
Note the record albums at which the pair are looking, which seem to be given just enough concentration to suggest there may be more to them than just musical entertainment. For example, Charlie holds the album cover which they choose to play facing backwards for several moments. Several photos can be seen, one of which almost resembles Danielle (who had a particular fascination with the French lyrics scrawled on her maps). In fact, it is this record that Hurley chooses and refers to the artist as "she".
Also brought to clear attention is the unknown "Geronimo Jackson", appearing to be dated back a few decades like most other hatch components.
Sayid enters, noting the "depressing" music that was playing, and asks about the whereabouts of Jack and Locke. Hurley reveals that they had gone in search of Michael-- including Sawyer, and Kate.

Of course, he then references to Jack the other, far less jovial metaphor about curiosity as a subsequent warning.

Around the "fireside chat" with Mr. Friendly, a brief discussion is held between the four men regarding the shrewd secrecy surrounding the kidnapper and alleged "Other" who claims to "come in peace". Mr. Friendly insists that Michael will not find them (hinting his knowledge of Michael's whereabouts?). He confirms that "Walt is fine" (much like Goodwin's response to the condition of the stolen children from the tail section). He also implies knowledge of the survivors by addressing them by name-- and refusing to explain how he came upon such information.
Jack refers to a connection between Ethan Rom, the infiltrated "spy" to the survivor camp, to which Mr. Friendly displays no immediate recognition nor confirmation. Instead he professes that this is "our island", and the survivors had been living for two months by his own discretion. (This further implies the relation to the DeGroots, as the island is likely Dharma-owned and possibly erased from known existence on the maps. The possibility of an intentional cause for the plane crash is questionable, considering the nature of the organization.)
He then proceeds to directly quote Alvar Hanso, adding a few "curiosity" metaphors and glancing accusingly at Locke regarding places which should not be explored and doors not intended for opening. (Thus, hatch connection.)
Jack refuses to believe, until his "theory" is dismissed by a yell to "light them up". Suddenly, the three survivors find themselves surrounded by torch-bearing "Others" (at least ten to twelve) in a unanimously glowing ring of fire.
Although she goes unseen, the existence of Danielle's long-lost Alex is presumably authenticated when she brings out the prisoner, Kate. (Now about sixteen, Alex's joining with the "Others" lends suggestion that kidnapping of children is insurance that their group will continue to maintain itself.)
Jack is left with a choice-- give up, or forfeit Kate's life. One by one, the three lay down their weapons: first Jack, with the resentment and anger in his eyes; then Locke, unable to conceal a second weapon from the alarming eyes of the stranger; and finally Sawyer, who vows that he and "Zeke" are not through yet.
Jack's extreme disappointment in being forced to give up the search for Michael permanently changed his former relationship with Kate, having been betrayed a second time.

With the work of antibiotics leaving him adequately cured, Sawyer once again returned to his characteristic name games with his fellow survivors. During the trek, Sawyer had appointed "Daniel Boone" to Locke while tracking Michael, as well as the humorous "Mr. Clean" (a commentary on the resemblance, adding that Locke needed only an earring and a mop).
Locke acknowledged his appreciation by revealing to Sawyer that he had learned Sawyer's real name through the flight manifest (which also explains Locke's sudden reference to Hurley as "Hugo", previously without explanation).
At the end of the search and upon their return to the beach, Locke departs with a "see you, James" (much to Sawyer's chagrin). Locke then approaches Claire and greets Aaron-- referring to himself as "Uncle Johnny" in the process-- while Charlie looks on with an unsettling expression.
Jack seems to have perplexing thoughts of his own, when he once again approaches Ana Lucía with grim news of the failure to locate Michael. Jack's determination apparently had intensified, not ceased, and he was now prepared to counter the forces of the "Others" with an army of their own...
