Holodeck 2 is located on deck 6, as we learn in "Twisted". It's funny, but we never learn where holodeck 1 is.
2. In "Worst Case Scenario", we see a training program about Maquis mutiny, written by Tuvok. What was the name of that program?

Answer: Insurrection Alpha

Tuvok created that program, and Torres later discovered it during usual computer purge. Seska made some modifications to the program and tried to kill Tuvok while he was in it.
