The problems came from poorly placed vents on the console which led to overheating inside the console and the warping of the internal plastic parts holding the game CD.

The first PlayStation consoles also used a laser unit whose case and all movable parts were made from plastic. Over time, friction and heat caused the plastic lens container to wear out unevenly. Eventually, one side of the lens container became so worn that the laser tilted and was no longer pointing directly at the CD. This tilting would cause data reading errors thus games would not load properly.
