1. Which squad member was severely wounded on Mars?

Answer: Ashley or Kaidan

In "Mass Effect", either Ashley or Kaidan were sacrificed to stop Saren. The one that lived returned to be a squad member in "Mass Effect 3". On Mars, Shepard started with James and either Ashley or Kaidan. The squad later met up with Liara, and James was sent back to the shuttle. Liara helped Shepard track Dr. Eva Coré, who had sabotaged the research facility to let Cerberus in. It turned out that Dr. Coré was an AI in an advanced robotic body. She severely wounded either Ashley or Kaidan just before Shepard and the squad got off Mars. They took the robotic body, and EDI commandeered it for herself, becoming a squad member.

Ashley or Kaidan were taken to the hospital on the Citadel. Once they had healed, Shepard had the option of letting them rejoin the Normandy's crew.

Tali, provided she survived "Mass Effect 2", did not appear in "Mass Effect 3" until Shepard went to resolve the quarian-geth conflict.
2. What was the name of the super weapon that the galaxy started building to destroy the Reapers?

Answer: The Crucible

The Prothean ruins on Mars contained vast databases. Liara and her team had managed to uncover plans to build an anti-Reaper superweapon that they called the Crucible. The Alliance began construction of the Crucible right away, and enlisted any who wanted to help.

Zhu's Hope was the name of the human settlement on the world Feros, which Shepard visited in "Mass Effect". The Lazarus Project was the name of the project to revive Shepard in "Mass Effect 2". Project Overlord was a "Mass Effect 2" DLC in which Cerberus scientists tried to combine a human brain with a geth AI as a means to control the geth.
3. What component of the super weapon was Shepard sent out into the galaxy to look for?

Answer: The Catalyst

The Crucible plans required something called the Catalyst in order to function. Nobody knew what the Catalyst was, and Shepard was sent into the galaxy to figure it out (along with uniting the races of the galaxy against the Reapers). At the very end of the game, the Crucible was attached to the Citadel to power it.

The Cipher was the essence of being a Prothean that Shepard acquired in "Mass Effect" to understand the visions from the beacons. The Conduit was a mass relay that was a back door onto the Citadel. In "Mass Effect" Saren used it to infiltrate the Citadel and take control of the station. Shepard followed him through, though, and stopped him. The Genophage was the salarian and turian solution during the Krogan Rebellions to curb the krogan's incredibly high birthrate.
4. Which group was holding the female krogan who had been cured of the Genophage?

Answer: The Salarian STG

The leader of the krogan, Wrex or Wreave, learned that there was a female krogan who had been cured of the genophage. They also knew that she was being kept by the salarians in one of their secret STG bases. Shepard traveled to Sur'kesh, the salarian homeworld, to retrieve the female and secure krogan support.

Cerberus attacked the STG base during the extraction. The turian hierarchy had an interest in Shepard recovering the female. They badly needed krogan support against the Reapers, and the genophage cure was the krogan's price. The geth were not seen in "Mass Effect 3" until after the genophage issue had been resolved.
5. Which species declined Shepard's invitation to a war summit?

Answer: Asari

Shepard called a war summit of the galaxy's representatives of different species in order to unite them against the Reapers and help Earth. Unfortunately, most species were busy dealing with their own issues, and had little desire to coordinate. Only the turians, salarians, and krogan showed up - a very unstable mix.

The turians needed krogan support to stop the Reapers from taking their homeworld. The krogan wanted a cure to the genophage, which they knew the salarians had. The salarians, who were not yet fighting the Reapers, saw removing the genophage as too great a risk, even to stop the Reapers.
6. Was it possible to see both Mordin and Wrex at the final battle on Earth?

Answer: No

Provided they survived the previous games, Mordin and Wrex would appear in "Mass Effect 3". If not, Wrex would be replaced by his half-brother Wreav, and Mordin would be replaced STG operative Padok Wiks. Wrex was the leader of a krogan clan, and was busy uniting the clans to give the krogan a stronger standing on the galactic political stage. Mordin returned to the STG to work on the reversing the effects of the genophage. He was the leak that let Wrex knew about the cure.

The salarians knew that someone might one day try to reverse the genophage using their own facilities. They sabotaged them to prevent sure a cure from working. In order to properly disperse the cure, the sabotage had to be reversed, and Mordin had to sacrifice himself in order to do so.

Shepard could also side with the salarian government, and choose to secretly let the cure be destroyed to still secure krogan support. Shepard would have to either convince Mordin to walk away, or shoot him. If Weave was leader the krogan, he would not notice the deception, and Shepard could acquire both krogan and salarian support. If Wrex was leading, though, he would eventually figure out Shepard's plot and confront the Commander. Wrex would die in the resulting fight.
7. Where was Kai Leng killed?

Answer: Chronos Station

Shepard first saw Kai Leng on the Citadel during the Cerberus attack. Shepard later fought Kai Leng on Thessia, where Leng escaped with the Prothian VI. Traynor tracked Leng to Horizon, and Shepard discovered Cerberus' sick experiments there. Shepard saw Leng on security cameras, but never actually met up with him. Finally, during the attack on Cerberus' Chronos Station, Shepard met up with Leng and finally killed him.

After defeating Cerberus at Chronos Station, Shepard was able to focus on the Reapers on Earth.
8. Who did Shepard have to bring to Thessia?

Answer: Liara

A few times in the game, Shepard was forced to take along certain squad members. Mars required Liara and the survivor from "Mass Effect". Menae forced Shepard to take Garrus, if he survived the previous games. Parts of the conflict around Rannoch required Tali to be brought along, if she survived.

When Shepard traveled to Thessia, the asari homeworld, Liara (an asari) insisted on coming. EDI, with her history with Cerberus, went with Shepard to the final Cerberus conflict at Chronos Station.
9. Was it possible to have both geth and quarian support at the final battle on Earth?

Answer: Yes

The quarians decided to take back their homeworld after 300 years of exile. The geth, in their desperation to survive, turned to the Reapers for support. The geth did this for survival, but did not like being having their independence compromised. Shepard freed the geth from Reaper control and helped them become true AI. The quarians were still distrustful and ready to destroy every last geth during their moment of weakness, and the geth were prepared to defend themselves. Shepard could either cripple the geth long enough for the quarians to destroy them, or do nothing and let the geth regain their advantage and destroy the quarians.

If Shepard was incredibly persuasive, and passed a large battery of checks, then the Commander could talk both sides down from destroying each other, and gain the support of both. To get this outcome, players had to import a Shepard from "Mass Effect 2" who had made some specific choices in that game.
10. Who made it onto the Citadel with Shepard at the very end of the game?

Answer: Anderson

During the final rush to the gravity lift with Hammer squad, Shepard's squad was either severely wounded or killed. Shepard sent the Normandy off with the survivors and went up the gravity lift alone. On the Citadel, Shepard discovered that Anderson had also made it up.

Shepard met Anderson and a very corrupted Illusive Man at the control center for a conversation. Both Anderson and the Illusive Man were shot. Shepard activated the Citadel so it could dock with the Crucible, and then sat down with Anderson to admire the view of Earth from orbit. Anderson shared a few parting words before silently fading away, leaving Shepard weak and alone.
