Answer: Frank Gusenberg was the survivor and he died at the Alexandrian Hospital, Chicago.

the theatre closed after the asssassination of President Abraham Lincoln on April 14, 1865 while watching the play "Our Amercian Cousin". On July 8, 1865 the War Department began leasing the building from Ford, converting the theatre into a three story office building. In December 1865 the Surgeon General was authorized to use the building as Army Medical Museum. From 1866 to 1887 it was a three story office building: War Department records on first floor, Library of Medicine on second floor, Army Medical Museum on third floor.
