The first time you fight him is at the 'turning point' of the game, when Kratos betrays you. Next you fight him after he explains his reasons for splitting the world, and tells you his plans. The last time you fight him in his Yggdrasill form is after you defeat Pronyma. In the final battle, you fight him in his child form, then when he is in a machine.

What item did Mithos use to split the world in two?

Eternal Sword. Mithos got the Eternal Sword from his pact with Origin. It was made so that only a half-elf could use it. After Zelos 'betrays' you, he gets you an item so that Lloyd can use the Eternal Sword even though he is not a half-elf.

Genis. Pronyma attacks Genis, and Mithos pushed him out of the way and takes the hit. In the proccess, Mithos drops his panpipe. This is when Genis first starts to suspect that Yggdrasill is actually his half-elf friend, Mithos.

What new age is Mithos trying to create?

Age of Lifeless Beings. Mithos thinks that the only way to stop discrimination is to make everyone exactly the same. If he creates an age of lifeless beings, so people will no longer need to be afraid of what is different. Cruxis and the Desians both exist to create this age.
