When wearing this disguise, Count Olaf had a fake nameplate. Who was he?
    Shirley. In book four, Count Olaf disguised himself as a receptionist named Shirley. On Shirley's desk was a nameplate that said, 'Shirley'. Count Olaf claimed that if he had a nameplate stating that his name was Shirley, then he was indeed Shirley.

In the "Hostile Hospital", Count Olaf was disguised as a veterinarian.

    a monocle. Count Olaf disguised himself as a German auctioneer in this book . The disguise required him to use a phony accent, wear a pinstripe suit, and to wear an eye monocle.

What was Count Olaf's fake name and occupation in "The Ersatz Elevator"?
    Gunther the auctioneer. Gunther was an auctioneer, who supposedly was from Germany and didn't speak much English.

Who was Count Olaf disguised as when he shaved his head?
    Stephano. In book "The Reptile Room", Count Olaf shaved both his head and eyebrows (actually he only had one eyebrow), along with growing a beard to disguise himself as a herpetological assistant named Stephano.

When Count Olaf disguised himself in "The Hostile Hospital", what was the name of the hospital he worked in?
    Heimlich Hospital. The Baudelaires joined the Volunteers Fighting Disease, who helped spread joy in this particular hospital where Count Olaf tried to perform brain surgery on Violet.
