Swan's mother was Darleen. At the beginning they are living in a trailer park with 'Tommy', another loser boyfriend of Darleen's.

Josh was a gentle soul who became Swan's protector.

McCammon left this book open for a sequel, but 'Swan Song' as well as King's 'The Stand', pardon the pun, stand alone. They have mythical, magical qualities that would be ruined by sequels. And, more importantly, you yourself can imagine any outcome for the survivors, and know in your heart that they made out alright...or possibly not.
