Elena was adopted and we didn't find out this information until season 3 when her biological father showed up with her half-siblings.

Daniel Santos wanted Elena for himself because she was the only known female werewolf in existence plus he hated Clay because Clay killed his brother.

Elena's biological father was named Sasha Antonov. She had no idea that she was adopted because she had always thought that her parents were the Michaels who had raised her as their own.

Logan and Elena had to hide from their human lovers Rachel and Phillip that they were werewolves because humans had to be killed if they ever found that wolves existed. When they were getting ready to face Jimmy Koenig who had been hired by Daniel Santos to kill them, Nick and Clay were talking about whether or not Jeremy would be able to fight him. That is when Nick made that statement.

Jeremy thought that Elena had seen him change from wolf to human and he wanted Clay to kill her in order to protect their secret. Instead of killing Elena like Jeremy had ordered, Clay disobeyed Jeremy's orders and bit her instead.

Elena's grandfather Roman had sent her father Sasha to watch over his daughter Natalya and protect her from harm. What he didn't count on was that they would fall in love and Sasha would show her his true form which led to Roman killing his own daughter. Both of them ended up blaming each other for her death.

Answer: Konstantin told her the truth.

Elena had no idea that Roman and Konstantin were actually her grandfather and uncle until the episode "A Quiet Dog" when she was guarding him and he was trying to figure out why her pack had gotten involved in the feud between Roman and Sasha. Konstantin figured out why Elena had survived the change and it was because she had born a werewolf.

Clay always loved calling Elena darlin' and that was his pet name for her. He called her that all the time. She called him that after she was attacked by the Albino because she was Sasha's daughter.
