Answer: Shane

Shane was a cop who served alongside his best friend, Rick. At around the time of Shane's demise, his friendship with Rick grew sour. Shane wanted to be with Rick's wife, Lori, but couldn't while he was alive. Shane brought Rick into the woods to look for an escaped prisoner they had (but in reality, Shane let him go and killed him so they could search).

As Shane was going to kill Rick, Rick stabbed him in the chest, killing him. Shane reanimated into a walker, and was put down by Rick's son, Carl.

2. During the season 4 mid-season finale, the Governor cut off somebody's head after a somewhat civil conversation with Rick. Who was the unlucky soul who was decapitated?

Rick stood up the Governor (who was accompanied by his small army and a tank), telling them that they could all live in the prison to avoid anymore bloodshed. The Governor called him a liar, and used Michonne's katana to slice through Hershel's neck. This was followed by a firefight that lasted the rest of the episode.

Lilly, the Governor's love interest, walked over to him near the end of the episode. As he began to cough up blood, she shakily raised a pistol and put him down.
