Answer: The Fourth Doctor

Tom Baker's Fourth Doctor wore the famous scarf in all but one of his episodes. Evidently it was never meant to be so long. Someone from Wardrobe was given some wool to knit "a scarf" and used the lot!
Colin Baker's Sixth Doctor wore a similar scarf at times, but he wasn't on the list.
2. One of the Doctor's companions wore a peculiar Andy Pandy outfit in one of her stories. Who was it?

Answer: Sarah Jane Smith in "The Hand of Fear"

Sarah Jane, played by Elisabeth Sladen, was the unlucky girl to masquerade as Andy Pandy. "The Hand of Fear" was her last "regular" appearance. (She was in "The Five Doctors" special and also in "K9 and Company".)
3. Who wore a school uniform for at least one story?

Answer: Turlough

Vislor Turlough (played by Mark Strickson) was pretending to be a public school boy in his first story, "Mawdryn Undead". He was at first a traitor infiltrating the Doctor's Tardis, but later became a friend.

Answer: Barbara

Barbara Wright (played by Jacqueline Hill), companion to the First Doctor, wore this gown in "The Keys of Marinus". While searching for the keys, she came to a place of illusions where even the Doctor was bluffed into thinking a battered enamel mug was a piece of scientific equipment.

Sharaz Jek, the deformed madman in "The Caves of Androzani" wore this get up. It was very effective.

The Fifth Doctor wore this jacket over something that looked like cricketing trousers. Apparently it's the correct garb for an Edwardian cricket match. He also wore a stick of celery on his lapel.

Wrack, played with enormous verve by Lynda Baron, wore the famous bodice. Wrack was an Eternal, one of a race of beings sailing ships through space. She was Captain of the ship "Buccaneer".

Answer: The Black Guardian

The Black Guardian, played by Valentine Dyall (1908-1985), wanted the Key to Time, and recruited Turlough to get it. He dressed in black, including the rather evil-looking headdress mentioned. He appeared in "Enlightenment", "Terminus", "Mawdryn Undead" and "The Armageddon Factor". The White Guardian, who wore a kind of dove hat, was played by Cyril Luckham.

Answer: Meglos, when he wasn't pretending to be the 4th Doctor.

Meglos, the last of the Zolfa-Thurans, looked like acactus in his natural form, but could change his appearance at will. He spent a while masquerading as the Fourth Doctor, but his spines eventually showed through.

Gebek was a moderate among the miners. He appeared in "The Monster of Peladon". The hairdo was unforgettable.

Katy Manning did this unlikely thing in 1977, posing with a Dalek for "Girl Illustrated" magazine. It was a tasteful photograph.

Ace, played by Sophie Aldred, wore this punk-inspired jacket during her tenure as companion to the Seventh Doctor. Ace's other trademarks were Doc Martins and a can of Nitro-9.

Liz Shaw (played by Caroline John) and Jo Grant (Katy Manning) both appeared in mini-skirts and boots. After all, it was the 1970s. Leela certainly wore short garments, but it wasn't a mini-skirt. And a mini wasn't Ace's style at all.

The Sea Devils wore very fetching mesh tunics that looked as if they'd been cut out with secateurs and tacked in place with fishing line. With the frilled-necked lizard heads sticking out they were strangely effective. Apparently the creatures reminded Jon Pertwee (The Third Doctor) of his little dog.

Styre, like the rest of the Sontarans, wore this particular collection. It was an unlovely get up, but when Styre took the helmet off, most viewers probably wished he'd left it on. Sarah Jane certainly didn't like it when she encountered Styre during "The Sontaran Experiment".

Leela wore her trademark "savage" garment for much of her time on the show. It appears to be a sleeveless leotard with a short apron at the front and knife sheaths at the sides. She had boots to match. She seems to have had two; one brown and the other a cream colour. The lighter one had arm bands.

Jamie McCrimmon (played by Frazer Hines), a young piper, joined the Second Doctor in his travels after the Battle of Culloden. He wore the famous kilt. (I presume it was McCrimmon tartan?) Frazer's first story was "The Highlanders" and on the strength of his acting (or maybe his kilt) he became a steady companion instead of a guest artist, as originally planned. Diana Gabaldon named her Scottish hero "Jamie Fraser" after Jamie (McCrimmon) Fra(z)ser (Hines).

Answer: Peri

In "Mindwarp", Peri (Nicola Bryant) is prepared for a brain transplant. She has her head shaved, and is dressed in a red and yellow (somewhat harem-guardish)outfit, with earrings. The big rescue scene doesn't happen, and Peri is taken over. Only later it turns out she married King Ycarnos.

Answer: The Seventh Doctor

The Seventh Doctor (Sylvester McCoy), carried this umbrella in some of his stories. It was introduced in "Delta and the Bannermen", and the Doctor found it very useful in "Dragonfire".

Answer: Adric

Adric, from Alzarius in E-Space, was a companion of the Fifth Doctor. Played by Matthew Waterhouse, Adric first appeared in "Full Circle" and died in "Earthshock". Adric was one of only two or three (depending if you count Peri) companions to die while associating with the Doctor.

The Fourth Doctor wore these Sherlockian garments for the Victorian gothic tale set in foggy London. The allusions were flying like fur at a catfight, and scriptwriter Robert Holmes must have had the time of his life.
