1. What is the very first thing that happens after the opening cutscene?

Answer: A bartender refuses to sell you a drink

When the game starts the bartender will not sell you a drink, even though you offer to make it worth his while. You then leave the tavern and then you are murdered.
On a sidenote you can kill this bartender during the game.

2. What is the very first town you enter?

Answer: Nactholm

Natcholm is also the town you are murdered in.

3. "Look to the pillars of _______ for an answer." What fills in the blank?

Answer: Nosgoth

Near the beginning of the game Mortanis informs you that although the actual assassins are dead (by your hand) someone else hired them and that you should go to the Pillars of Nosgoth to seek answers.

4. Who is the first boss in the game?

Answer: Nothraptor

Nothraptor is the first boss you meet.He was driven insane by the murder of his love, Ariel (who you meet beforehand at the Pillars), and used his magic to infect the minds of the entire circle except for Kain.

5. How many pillars are there?

Answer: Nine

There are nine pillars and you must hunt down the eight members of the Circle and kill them, after which you bring their artifact to the pillars and restore them.

Answer: The Oracle of Nosgoth

Mobius used his guise as the Oracle to trick Kane into killing William the Just. By changing history Mobius was able to start the vampire purge, ending with the decapitation of Vorador. They tried to kill Kain but failed.

The Heart of Darkness is the first artifact you find and Flay is the second.

As far as I know your state of health does not determine whether you can enter anything. You use the mist form to go through doors and bars without opening them.

After you defeat Mobius a cutscene will play showing Kain cut his head off. Kain never dances to celebrate and if you thought Kain was going to let ANYONE live you really need to play this game more.

Answer: To rule Nosgoth and condemn it or die and save it

Kain never gets the choice to leave Nosgoth, Ariel is already dead and Heaven and Hell are not mentioned.
