"Surfacing" by Trust Company was on "MVP Baseball 2004". "MVP Baseball 2005" doesn't feature it.
2. What former second-baseman is the play-by-play announcer?

Answer: Duane Kuiper

Duane Kuiper only hit 1 home-run in his career. He hit it off of Steve Stone.
3. Who is on the cover?

Answer: Manny Ramirez

Albert Pujols was on the "MVP Baseball 2004" cover. Randy Johnson was on the "MVP Baseball 2003" cover along with Miguel Tejeda. Javy Lopez has not been on an "MVP Baseball" cover as of 2006, and because of the yearly releases, it's likely he may never be on it.
