Hyperion. While the exterior shots of the building are actually of a real hotel in Hollywood called the "Los Altos Hotel and Apartments", some interiors where taken from the famous Ambassador Hotel in Los Angeles. It opened in 1921, but was closed down in 1989. After years of legal struggles, the hotel was demolished somewhere close to/in 2005.

Joss described the reason for this on his commentary for the episode, and the reason it had to be removed from the final cut. His entire inspiration for the episode was to have Amy and Alexis dance, and he had to cut it out!
The Nyazian Prophecies. This caused a huge weight to be placed on Wes' shoulders, which resulted in him visiting the Loa at Jolly Burger and then taking Connor away. We later learn, straight from the horse's mouth, that Sahjhan faked the prophecy to save himself from Connor.
Quor'toth. He raises Connor to hate Angel and he renames him "Stephen". He had originally planned to go to Utah with Justine, but you can thank Lilah and the Wolfram and Hart crew for Connor's trip to Quor'toth.
No. Holtz killed himself. He saw that Angel and Connor were starting to bond and so he had to do something to make sure that Connor would never trust Angel. He tried to get Justine to do it but she loved him too much to kill him. He took the tool that he wanted her to kill him with and stuck into his neck to make it look like a vamp bite. She added the second wound, after his death. When Connor found Justine holding Holtz' dead body, she told him that it was Angel who attacked and killed Holtz. Connor believed her and then promised vengeance against Angel. Meanwhile, Holtz had previously led Angel to believe that Connor belonged with him. Holtz wanted Angel dead, and he knew that Connor loved him enough to kill for him. But instead of killing Angel, Connor and Justine locked him in a wooden box and sent him to the bottom of the ocean, where they believed that he would suffer forever.
No. She raises into a higher plane instead, and Connor fights with Angel at Point Dume. After Connor has his father incapacitated, he signals to Justine's boat and the duo weld him into a steel coffin. They send the coffin deep down into the ocean, as Cordy is seen rising into the heavens.
