1. Ice Rapier

Answer: Frozen Eleum Loyce

Well known for being one of the most hated weapons in the game's history, the Ice Rapier is dropped by a Rampart Soldier found shortly after the area's second bonfire going up a small flight of stairs. The weapon is extraordinarily powerful in Player-versus-Player combat.

This is because, when a strong attack is used, the rapier thrusts forward a Soul Spear that costs very little durability and is exceptionally powerful. Both the weapon itself and Soul Spear can deal damage simultaneously, making them a devastating combo in close quarters.

The rapier itself is also very fast, sharing the same moveset as other rapiers. Finally, it loses virtually nothing upon being infused while also gaining the desired infusion, making it a fantastic candidate for infusion and making it nearly unbeatable, barring a large difference in skill level or the opponent also using the weapon.
2. Grave Warden Agadayne

Answer: Undead Crypt

Agadayne can become both a vendor and a helpful summon for the area boss in Velstadt the Royal Aegis. He will become hostile towards the player if, after demanding they turn a torch off, they refuse.
3. Straid of Olaphis

Answer: Lost Bastille

The player will need a Fragrant Branch of Yore. If they have one handy, they must head to the top floor of the Lost Bastille prison and use it on Straid's petrified body. Upon doing so, the Straid's Cell bonfire becomes accessible and Straid will now convert some boss souls into weapons and sell the player some items.
4. Guardian Dragon

Answer: Aldia's Keep

The Guardian Dragon is the boss of Aldia's Keep. Its boss soul can be converted into the Spitfire Spear, a spear which deals split physical/fire damage.
5. Ancient Dragon

Answer: Dragon Shrine

Ancient Dragon is an optional boss of Dragon Shrine. It is not initially hostile when approached, and will give the player the Ashen Mist Heart when talked to. In order to trigger the fight, the player must simply attack the dragon. If they do so but are later defeated, the dragon will now immediately be hostile if the player returns to its boss room.
6. Sinh the Slumbering Dragon

Answer: Shulva, Sanctum City

Sinh is the final boss of the "Crown of the Sunken King" DLC area in Shulva, Sanctum City. He is fought directly after Elana the Squalid Queen. His soul can be converted into the Yorgh's Spear, an extremely powerful spear scaling mainly off of both Strength and Dexterity.
7. Santier's Spear

Answer: Doors of Pharros

The Santier's Spear is unique for how it interacts with durability. When the weapon is 'broken', it will actually transform into a completely different, significantly more powerful Twinblade. This also means it has functionally infinite durability in this state.
8. Gower's Ring of Protection

Answer: Drangleic Castle

This ring significantly reduces any damage the player takes from behind. This includes backstabs. It will break after taking a handful of hits from behind however, so this isn't a free pass for the player to ignore their positioning, but rather an insurance policy for if a mistake is made in this regard.
9. Heavy Iron Key

Answer: Forest of Fallen Giants

The player can obtain this key in two different ways. The most simplistic would be to simply use the Soldier's Key to open a locked door in front of the Last Giant's boss room. Doing this will allow the player to walk in a straight line for about five seconds to pick up the key. Alternatively, they can drop down from the bonfire, kill a few Fire Salamanders, journey into their makeshift cave and grab the key. Doing this may seem impractical, but the player can loot additional goodies such as the Hawk Ring or Rebel Greatshield along the way, potentially making it worth the extra trek.
10. Giant Lord

Answer: Memory of Jeigh

The Giant Lord is the boss of the Memory of Jeigh. This is an overall extremely small area, as it takes about ten seconds to make it to his boss room when beginning the area, and there's even less to explore in the vicinity after the Giant Lord has been defeated.
11. Chaos Storm (Pyromancy)

Answer: Old Iron Keep

The player has a small shopping list of things to retrieve in order to access this spell. They'll need the Flash Sweat spell, the Fire Quartz Ring, a Red Troch and, while not strictly mandatory, a Pharros Lockstone would be helpful. These items are needed to ensure the player will not die trying to reach the treasure chest containing this spell, as it's surrounded by active magma.
12. Soothing Sunlight (Miracle)

Answer: Majula

This one is a tricky one to get. The player must first obtain the Crushed Eye Orb from the Undead Crypt, an item which is in itself a pain to get. Afterward, they must approach Licia of Linheldt in Majula and use the orb while standing next to her. This will cause the player to invade Licia, and killing her will bring them back to Majula, where she will now be dead and will have dropped the Soothing Sunlight miracle.
13. Dark Greatsword (Hex)

Answer: Cave of the Dead

Upon completely clearing the Cave of the Dead and defeating the boss, the player can proceed through the boss room and will be rewarded with numerous treasure chests in what is essentially a shortcut that will return them to Shulva, Sanctum City. One of these chests will contain the Dark Greatsword Hex.
14. Carhillion of the Fold (summonable NPC)

Answer: Brume Tower

Arguably the weirdest instance of a summonable NPC ever, Carhillion can be summoned to help fight Fume Knight, and his sign will be found right in front of the boss door. It will only appear here if the player has managed to get Carhillion to move from No Man's Wharf into Majula, achieved by having at least 8 Intelligence, defeating the Flexile Sentry and then speaking to Carhillion. Why Carhillion is at the Brume Tower is a mystery, seemingly almost selected by a From Software executive blindly throwing darts at an NPC dart board or the like.
15. Eleum Loyce (Curved Sword)

Answer: Grand Cathedral

Either by killing Alsanna, Silent Oracle or offering her fifty Loyce Souls, she will give the player her soul which can be converted into the mentioned curved sword.
