4. What is the name of Berlioz's opera?

Answer: La Damnation de Faust

5. What is the name of Boito's opera?

7. On whom does Mephisto put a curse?

Everything he touches will die. He figures it out when Marguerite's bouquet withers, and ends it by dipping his fingers in holy water. Mephisto also made Valentine's sword ineffective, but he wasn't one of the choices.
8. In retort to Brander's song about a rat, about what insect does Mephisto sing?

Answer: a flea

C'mon now, sing it with me... Une puce gentille Chez un prince longeait. Comme sa propre fille, Le brave homme l'aimait, Et, l'histoire assue, A son tailleur un jour Lui fit prendre mesure Pour un habit de cour.
9. Who did not play Mephisto?

Answer: Caruso

The other three did play him. If you didn't know that, you should know that Caruso was a tenor, and Mephisto is always sung by a bass or baritone.

Answer: Yes

In 'La Damnation de Faust', Faust dies. In 'Faust', Valentine, Marguerite, and Martha die. In 'Mephistopheles', Faust and Marguerite die.

12. In which opera is there a ballet section including Mephisto's conjuration of Helen of Troy (the more famous one)?

Answer: 'Faust'

Known as 'Walpurgis Night', it is often omitted from modern productions. I have also been notified by some one who has actually SEEN "Mephistopheles" that there is a ballet in that one too, and I didn't know about that, which is why I've noted the Gounod "the more famous one."
13. What does Mephisto wear in his cap in most costumes?

Answer: feather

In the good-ole-days, he usually wore one, from what I've seen in my research.
14. What is the last song in 'Faust'?

Answer: a trio

It's a real showstopper, starting with Mephisto's, 'Alerte, ou vous etes perdue!'
15. A play based on this story was written in the 1590's, "The Tragical History of Doctor Faustus". Who wrote it?

Answer: Christopher Marlowe

Marlowe was second only to Shakespeare as a dramatist of his time.
