Answer: A hospital orderly injected him, and he went into cardiac arrest.

The orderly was working for General Juma, and he spoke with Juma soon afterwards and reported that Dubaku was dead. Renee Walker was at the hospital and was stunned because the doctor had told her that Dubaku was going to recover. She investigated and found a picture of the orderly from a security camera, and after determining that he didn't work there, she found another picture of him leaving the hospital.

Answer: She had erased a name from Colonel Ike Dubaku's list.

Janis Gold had noticed that a line was missing from the list of government conspirators obtained from Colonel Ike Dubaku. She brought it to Chloe O'Brian's attention, but O'Brian didn't seem concerned, which made Gold suspicious. She monitored O'Brian's phone calls and heard her telling Jack Bauer that someone in the FBI had realized that she had deleted the line. Gold told Larry Moss, and he had O'Brian taken into custody so that he could figure out what she was up to.

She followed the men when they boarded a boat, and as it was pulling out, she jumped on the back of the boat. She saw General Juma showing his men blueprints of the target, and when they left the boat, she looked at the blueprints and saw that they were of the White House.

General Juma and his men had dived off the back of the boat and swam underwater to an underground tunnel that led to the White House. When they reached the White House, they burned through two metal gates and knocked down a wall to reach a basement room. One of General Juma's men was working in the White House as a repair man, and he let the rest of the men in through a hole behind a cabinet.

When General Juma saw the Secret Service reinforcements entering the White House, he decided to bluff them and announced that he had the president and would kill her if the agents didn't retreat. The president was actually safe inside a lockdown room behind a steel door, but the Secret Service had no way of knowing this.
