Answer: 12th Doctor

This comes from the episode "Twice Upon a Time" just before the 12th Doctor's (Peter Capaldi) regeneration into the 13th Doctor (Jodie Whittaker). After being electrocuted and shot by multiple Cybermen and caught in an explosion, the Doctor purposely delays his regeneration (as he doesn't want to change) only accepting his "death" after a meeting with his first form.

Answer: Rory Williams

This comes from the episode "The Angels Take Manhattan" just as Rory sees a headstone with his name on it. Seconds later, he is taken by a Weeping Angel. This feeds into the show's lore that states if you read something about the future, it will come true.

Answer: 10th Doctor

This is from the episode "The Stolen Earth". This is when the Doctor has been struck by a Dalek beam and is regenerating in the TARDIS with Jack, Rose, and Donna. However, he concentrates his excess regeneration energy into a spare hand he has, successfully maintaining his current form but healing from the injury.

Answer: 11th Doctor

This comes from the Christmas special "The Time of the Doctor" as he is regenerating into Peter Capaldi's Doctor. After living for hundreds of years on Trenzalore protecting the town, the Doctor is minutes away from dying permanently (as he has no regenerations left) when Clara convinces the Time Lords trapped in another side of a split in time and space to help him.

They give him a whole new regeneration cycle.

Answer: 9th Doctor

This is from the episode "The Parting of the Ways" as Christopher Eccelston's Doctor regenerated into David Tennant's. After Rose looks into the heart of the TARDIS and defeats the Daleks, the Doctor takes the power out of Rose (as it is killing her) and puts it back into the TARDIS. However, the power is ultimately too strong for him and causes the first regeneration of the new series.

Answer: 10th Doctor

This is said in "The End of Time" just before the Doctor regenerates into Matt Smith's Doctor. After defeating the Master and sending the Time Lords back to where they belong, the Doctor is fine. However, Wilf Mott (Donna's grandfather) is trapped in a radiated case where the only way to set him free is for the Doctor to take his place.

After doing so, he visits the companions we've seen so far in the show, seeing that Martha and Mickey are together and happy, Sarah Jane and her son are doing good, watching Donna get married and handing her a winning raffle ticket, and going into Rose's past to see her before their initial meeting.

He then regenerates.

This is said not long after Rory Williams' last words. After seeing the gravestone and deciding that she doesn't want to live a life without her husband, Amy says goodbye to River Song and then takes her eyes off of the Weeping Angel that took Rory and says goodbye to the Doctor just as the Angel takes her.

Answer: The TARDIS

This is said in "The Doctor's Wife" just as the TARDIS leaves Idris' body and returns to the TARDIS shell. After the TARDIS matrix is forced into a human body (named Idris) and the Doctor, Idris, Amy, and Rory defeat the people responsible, the human body quickly begins to die.

The Doctor and the TARDIS have never been able to talk in the traditional sense and the encounter they've had effects them both. While the TARDIS may not actually be dying, the body is dying and this is the last chance for the two to communicate like this.

Answer: River Song

This is said in "Forest of the Dead", the second part to the episode "Silence in the Library". The Doctor and the team discover a way to free all the people trapped into the CAL server but it could potentially kill the Doctor. In an effort to save his life and their future that he hasn't had yet, River knocks the Doctor, handcuffs him, and puts herself in the machine that will save the people.

The Doctor wakes and pleads with her not reconsider, but she refuses. When the Doctor reveals that she whispered his real name to him and questions when and how and why he did so, she says her last words and activates the machine.

Answer: War Doctor

This is said in "Day of The Doctor". After defeating the Zygon invasion and deciding (along with his older selves) that rather than destroying Gallifrey and all the people on it they should instead time lock it in a Time Lord painting, the three Doctor's return to their TARDISes where the War Doctor regenerates into the 9th Doctor.
