1. Chakotay is walking through a corridor onboard Voyager when he gives a voice command to the ship's computer to locate Janeway; that command is promptly belayed before he is then ordered to enter the captain's quarters.

Chakotay suspects that something is wrong with his captain since she and the Doctor left Voyager to go on a brief away mission together. And he's right, although he doesn't know that Janeway has really been taken captive by the 'spy aliens' while the Doctor has been impersonating her.

In one scene, Chakotay stops at Janeway's quarters and asks the ship's computer to locate Janeway. Before the computer can respond, however, the Doctor (posing as Janeway) tells the computer to belay that order before saying to the commander, "My quarters--now!", their discussion then proceeding to continue behind closed doors.

In "Displaced", even with all of the crewmembers disappearing from the ship one by one, and Chakotay surely curious as to their unknown whereabouts, he's never asking the ship's computer to locate Janeway.

In "Night", Chakotay is already well aware of the captain's whereabouts: in her own quarters, which was her preferred place to stay while Voyager was traversing the starless region nicknamed 'the void'.

Then in "Equinox, Part 2", it's true that Janeway, in her determination to apprehend Captain Ransom, was flying solo in her decision-making and belayed every order that Chakotay gave. But again, Chakotay is already well aware of the captain's whereabouts, approaching her on many occasions to question her authority, and thus has no need to inquire the ship's computer for that information.
2. Ensign Wildman is seen noticeably pregnant for the FIRST time in this episode.

We learn of Samantha's pregnancy at the very end of "Elogium", but we don't actually see her stomach starting to show until she checks in for her prenatal visit to sickbay in "Tattoo". Wildman does not appear at all in neither "Cold Fire" nor "Resistance".

She does, however, appear again in "Dreadnought" for her second prenatal visit, but this episode aired much later than "Tattoo", and thus cannot qualify as the correct answer here.
3. Ensign Vorik is seen verbally interacting with at least one other crewmember OUTSIDE OF engineering in all of the following episodes...except this one.

Although the recurring Vulcan was most often seen assisting B'Elanna in engineering, there were occasional instances when he did get to venture to other areas of the ship and beyond. In "Counterpoint", we do see Vorik when he materializes in the cargo bay, after being released from transporter suspension along with Tuvok and the Brenari passengers.

However, Vorik says nothing at all in that scene; in fact, has no spoken lines in this episode whatsoever. In "Blood Fever", Vorik is seen speaking with various crewmembers in many places outside of engineering as he is dealing with his pon farr, including Tuvok's quarters (to get guidance from the wise, age-old superior) and on the surface of the Sakari homeworld itself (when he's partaking in the ritual combat with B'Elanna).

In "Alter Ego", Vorik appears on the holodeck briefly when he attends Neelix's luau, offering to dine with B'Elanna while Paris waits for Harry to arrive. Then in "Extreme Risk", after B'Elanna has had her heart to heart with Chakotay, she joins the away team on Voyager's new shuttlecraft, explicitly telling Vorik, who had been helping with the finishing touches, that he's in her seat, proof that the half-Klingon engineer was back to her old self.

Tuvok and the four ex-Maquis misfits are running laps through various parts of the ship as part of their Starfleet training. In one instance, the Bolian named Chell is noticeably lagging behind as they all run through the mess hall. Chell stops to ask a female crewman if he can have her glass of water (surely for a quick hydration boost), and when she allows him to, Chell proceeds to drink while still tiredly jogging. Unfortunately for him, however, Tuvok is standing at the doors on the opposite end of the mess hall with his hand out, and the disappointed Bolian hands over the glass of water to his Vulcan instructor before they both exit the room. No other such occurrence happened in the other three episodes.
5. Paris and Neelix entertain two rounds of the old 'shell game' on the mess hall countertop, the Doctor guessing correctly the first time but being tricked by them the second time.

Answer: Live Fast And Prosper

In an attempt to demonstrate to themselves that they haven't lost their touch of always being on top of their game and not being tricked so easily, Neelix and Paris set up a couple of rounds of the 'shell game' using three coffee mugs and a nut on the mess hall counter, purposely performing it in plain view of the Doctor.

In the first round midway through the episode, their attempts to trick the Doctor fail when the EMH successfully reveals that the nut is being hidden in the palm of Neelix's hand.

In Round 2 at the end of the episode, the Doctor DOES get tricked when Paris reveals that Neelix handed the nut off to him, much to the Doctor's disappointment. The 'shell game' is not seen being played in the other three episodes. On a side note, this scene always reminded me of the classic pricing game of the same name played on "The Price Is Right", bringing back fond memories of the days of former host Bob Barker.
6. Tuvok is referred to as a 'freakasaurus' by two different people in this episode.

Answer: Future's End, Part 2

Paris is riding around in the van through the streets while chatting about his experiences since Voyager's arrival in L.A. with Rain Robinson, his new-found babe who works at Chronowerx, the headquarters where Henry Starling's time ship is initially located. Rain candidly gives her free-spirited opinion about the three men she's befriended so far--Tuvok, Paris and the Doctor--explicitly calling the Vulcan chief of security a 'freakasaurus'.

Then later, when the senior staff is giving a toast to the future, Paris and Tuvok are recounting their experiences down in L.A., Voyager's conn officer explicitly asking his Vulcan crewmate if anyone has ever told him that he is a real 'freakasaurus'. Even with the omnipotent Q always up to their comical antics, Tuvok is not referred to by that silly nickname in neither "The Q And The Grey" nor "Q2", and the same holds true for the seventh-season episode "Inside Man"..
7. Icheb finds himself trapped behind a forcefield while onboard a vessel away from Voyager.

Answer: Q2

Icheb and Q's mischievous brat of a son are onboard the Delta Flyer together for the second time, following Paris's piloting lessons earlier in the episode. After Icheb has verified that a minor energy fluctuation has been fixed, Q's son powers up the shuttle's engines and creates an anomaly to venture to another star system, claiming that he's become bored and fed up with Starfleet rules and regulations.

When Icheb protests and tries to approach from the aft section, a forcefield blocks his way temporarily. That forcefield is promptly lowered as the alien ship (later revealed to be piloted by Q in disguise) fires upon the Delta Flyer, Q's son then realizing that he actually needs his ex-drone buddy to man the shuttle's tactical station to thwart off the attack.

In "Child's Play", it's true that Icheb was onboard the Brunali vessel, a plan devised by his parents to infect the Borg collective with the pathogen. But Icheb wasn't trapped behind a forcefield; he is successfully beamed back to Voyager once the crew is able to disrupt the tractor beam of the Borg ship that emerged from the transwarp conduit. Icheb remains on Voyager for the entire duration of "Imperfection" (only Paris, Janeway and Tuvok leave Voyager when on the away mission to retrieve the cortical node) and "Body And Soul" (only Harry, Seven and the Doctor leave Voyager, all of them getting captured by the Lokirrim).

After the trajector technology acquired from the Sikarians is destroyed due to incompatibility with Voyager's systems, B'Elanna tells Seska that everyone involved is going to take responsibility for their actions. Seska then objects, explicitly telling her former Maquis friend that she's changed, to which B'Elanna then accepts as a compliment.

In "Parallax", Seska (along with another male crewmember, presumably an ex-Maquis) does tell Chakotay that she'll help back him up if the Maquis wanted to take control of Voyager, then later, tells B'Elanna that she should've broken more than just Carey's nose.

Besides this, Seska has no other speaking lines, and certainly nothing along the lines of telling anybody that he or she has changed in character.

The same holds true for both "Investigations", when the Kazon have captured Paris, and "Maneuvers", when the Kazon have captured Chakotay; she doesn't tell either of them, or anyone else, that they have changed in those respective episodes.
9. Kes is the lone passenger onboard a shuttle that is seen being launched remotely from Voyager's bridge.

Answer: The Gift

With her heightened, uncontrollable powers causing damage all over the ship and putting the crew at great risk, Janeway orders Chakotay, who is on the bridge, to prepare a shuttle for their Ocampan friend so that she can leave Voyager. After Kes arrives safely in the shuttlebay (not seen on screen), we see the shuttle disembarking from Voyager, followed by Chakotay and the rest of the bridge crew operating the computers at their respective stations before he confirms that the shuttle has been launched. We next see Kes from the interior of her shuttle, glowing and fading in and out of phase as she gives her memorable farewell statement, "My gift to you all." In the sixth-season episode where she returns with a vengeance, we do see Kes as being the lone passenger on her ship when she is intercepting Voyager at the very beginning and at the end (after time is reset back to the original first encounter).

But we do NOT see her ship being launched remotely when she disembarks from Voyager for good.

In "Warlord", we do see a possessed Kes onboard a stolen shuttlecraft, but she is onboard with her Ilari accomplices as they are en route to the Ilari homeworld, where she temporarily assumes Tieran's role as the new autarch. Then in "Darkling", although we do see Kes on the surface of the planet of the Mikhal Travvelers, we do NOT see her onboard a shuttlecraft at any time at all.
10. Lieutenant Carey is seen verbally interacting with at least one other crewmember while working in engineering in all of the following episodes...except this one.

Answer: Friendship One

This one requires a little concentration, considering that Carey's appearances on the show were minimal after midway through Voyager's first season. "Friendship One" is best remembered as the episode where Carey is killed by weapons fire at the hands of the aliens on the nuclear winter-devastated planet. Ironically, the recurring Starfleet lieutenant who had previously competed with B'Elanna for the role of chief engineer is never seen in engineering at all in this episode.

He first pops up in sickbay where he, along with Neelix, Paris, Harry and Chakotay, receives his pre-flight treatment from the Doctor. We next see him onboard the Delta Flyer, and then ultimately, on the surface of the alien planet itself. "Relativity" is remembered as the episode where Carey is seen onscreen after four seasons of having 'disappeared', only being mentioned prior to that in "Before And After", when Paris talks to the future Kes about the events of 'The Year Of The Hell'.

In Seven's time jump back to the point where Voyager is stationed at Utopia Planitia at the very beginning of the episode, Carey is seen briefly operating controls at a computer console in engineering when he formally introduces himself to the unbeknownst-to-him futuristic ex-drone. Rewinding back four seasons prior to that, "Prime Factors" was the episode where Carey, along with B'Elanna and a soon-to-be-vengeful-Seska, covertly plot to obtain the trajector. All three of them are seen collaborating on their 'mini-conspiracy' in engineering while trying desperately to get the Sikarians' technology to function properly.

Then in the series pilot, we see Carey in engineering for the very first time. He's working with Janeway while trying to prevent the warp core from breaching after the Caretaker's displacement wave has caused massive damage all over the ship and rendered at least three of Voyager's original crew dead.
