The haunted old asylum was located in Rockford, Illinois. What was the name of the place?

    Roosevelt. Roosevelt Asylum had been established in either 1873 or 1878, as the date was a bit worn on the outside of the building. In the years since its closing, teens had intermittently broken into it to explore. A mistake in continuity occurred when Dean read from his dad's journal that no killings had occurred in the midst of the hauntings, until now. Then, later he stated that three kids broke into the asylum 1972, and one had died. The "now" killings Dean mentioned were of police officer Walter Kelly and his wife.

Who were the two policemen that entered the asylum at the beginning of the episode?
    Danny and Walter. Danny Gunderson and Walter Kelly went into the building to retrieve the most recent trespassing teens. Danny found the three kids hiding, but poor Walt made his way down into the basement. The door to the basement was ominously marked with the warning: Caution Biological Hazard. The evil spirit was able to influence Walter, who went home and killed his wife and then himself.

Sam called several people to try and locate their father John Winchester. He mentioned three names to Dean in regards to who he had called. Which was NOT one of the people Sam called to locate their father?
    Roosevelt. Unfortunately, none of these people had heard from John. Shortly afterward, Sam got a text message from an unavailable source. It contained the coordinates 9-52. These lead the men to Rockford, and Sam was convinced it was their father who'd sent the message.

While talking to Sam, Dean compared three TV psychics. Which was NOT one of them?
    John Edward. Dean jokingly asked his brother which psychic was "hotter", Patricia Arquette (who plays real life psychic Allison Dubois on "Medium"), Jennifer Love-Hewitt (Melinda Gordan on "Ghost Whisperer"), or Sam himself (Jared Padalecki). John Edward was not one of the many real life psychics, actors or characters mentioned during this episode. He also called Sam 'Haley Joel' in reference to Haley Joel Osment's character Cole Seer in "The Sixth Sense", who could also see dead people.

What was the name of the psychiatrist Sam went to visit, in order to obtain information on the events that closed the haunted asylum?
    James Ellicott. Dr. James Ellicott was the son of the doctor who'd run the asylum at the time it had been closed. Sam found out that the man's father, Sanford Ellicott, had been the Chief of Staff in 1964. A riot broke out that year, and many patients and staff were murdered. The asylum had been closed down, though Ellicott's body never recovered. Jack Torrence was the character played by Jack Nicholson and then Peter Webber in both "The Shining" movies. Dean mentioned two of Jack's roles in this episode, the other was his role in "One Flew Over the Cuckoo's Nest".

After learning about the 1964 riot inside the asylum, the Winchester boys went back. The encountered two teenagers inside. What were the names of these two kids?
    Gavin and Kat. Gavin and Kat (short for Katherine) were supposed to be on a date to see a movie, but Gavin thought exploring the asylum would be more fun. He was wrong. Kat was played by Brooke Nevin, who'd portrayed Nikki in season one of the "The 4400".

On their final visit to the asylum, who used the video camera to view their environment.
    Sam. Sam used the videocam, while Dean used his little ghost finding gadget. Sam first saw one of the dead patients through the camera lens.

When the teenage girl was locked in an exam room with one of the ghosts, she learned a number useful in the search of the asylum's evil entity. What was that room number?
    137. Based on his conversation with Gavin, and his own experience with the female ghost, Sam was sure that Kat had to face down the ghost in order to get out of the exam room. So, she stood very still while the man came close and whispered into her ear. She came out and told the men it was number 137, and they assumed it was a room number. This led to the brothers splitting up. Sam was supposed to get the kids out of the building while Dean went looking for room 137.

Sam received a fake cell phone call that lead him down into the basement.
    T. He was not able to get the kids out of the asylum, so he waited with them while Dean searched out room 137. While there, his cellphone rang and he heard Dean's voice calling him down into the basement. The call was not really from Dean, who had discovered Dr. Ellicott's secret experiments in room 137. The kids informed Dean that Sam had gone down into the basement looking for him, so Dean followed.

How was Dean able to get rid of the evil entity and set the asylum free?
    burn the bones of the doctor. First Dean had to fight off his infected brother, and then resist the influence of Sanford Ellicott's ghost. He'd already found the man's body, soaked it in lighter fluid and sprinkled salt on it. All he needed to do was set fire to it, and the evil spirit would vanish. He was able to fire up a Zippo style lighter and fling it into the cabinet that contained the body. At the very end of the episode, Dean's phone rings. When Dean does not wake up to answer it, Sam grabs it up. The episode ends with Sam asking if it was his dad that was calling. It must have sounded like John Winchester, by the hopeful look on Sam's face.
