Answer: Wisconsin

In this series the location is printed on-screen as the men arrive at their destination. This is similar to an older hit series "The X-Files", as well as another 2005 debut "Bones".
2. "Dead in The Water"- The angry spirit was that of a child who'd been accidentally killed 35 years earlier. Who was it that was responsible for the death of young Peter Sweeney?

Answer: Both of these men

Peter had been picked on all those years ago. Bill and Jake, who was the current sheriff, had held his head underwater until he drowned. They had not meant to kill Peter, but their cruelty had resulted in manslaughter. So, they allowed his body to sink into the lake and they hid is red bicycle. Bill's godson Chris had been one of the recent murders, due to Peter's vengeful spirit, and Chris' young son Lucas had witnessed the entire thing. Lucas hadn't spoken a word since the murder of his father, but had been drawing pictures that were clues to the murderer's identity.

These drawings led Sam and Dean to Peter Sweeney and finally to the connection between Bill Carlton and Jake Devins. Bruce Dawson played Bill Carlton and Daniel Hugh Kelly played Jake Devins. Kelly played Mark McCormick on the 1980s series "Hardcastle and McCormick".
3. "Phantom Traveler"- The first plane ride and the last plane ride all take place on which fictional airline?

Answer: United Britannia Airlines

This airline seems to be a combination of the real United Airlines and Britannia Airways. The first flight, #2485, went down when a possessed passenger opened the right rear emergency exit. The door flew open and damaged the plane's tail section, sending all but seven passengers to their deaths.
4. "Phantom Traveler"- Finish this phrase used by the demon-possessed people while in flight. "Now, time really does ____."

Answer: fly

The demon would happily state this phrase before ending the flights it was in charge of. He said this while in the body of victim number one, George Phelps, and again inside pilot Chuck Lambert's body. Chuck was the surviving pilot of United Britannia flight #2485, but did not escape when he piloted a small plane a couple months after the first crash.

The last human inhabited body was that of the co-pilot on the final UBA flight, #124, that carried Sam and Dean and flight attendant Amanda Walker. Amanda was the last survivor of UBA flight #2485 that the demon had the chance to kill, but it failed. Sam performed an in flight exorcism on the co-pilot and saved them all.
5. "Phantom Traveler"- Sam and Dean found and spoke to which survivor of flight #2485, at the Riverfront Psychiatric Hospital?

Answer: Max Jaffe

Max Jaffe was the passenger sitting behind George Phelps, a dentist on his way to a convention in Denver. George was demonically possessed and Max witnessed him do the impossible task of opening the emergency door while in flight. It was said that the door had approximately 4000 pounds of pressure holding it closed while the airplane was up. Max was able to see that George had black eyes, and that he was able to open that door with no problems at all. George was sucked out of the airplane instantly. Kett Turton played Max Jaffe (sounds like coffee).

He has also guest starred on "24", "Dead Like Me" and "Millennium".
6. "Bloody Mary"- Which life did the Bloody Mary claim first?

Answer: Mr. Shoemaker

Lily Shoemaker and her friends decided to play to Truth or Dare. They were all staying the night at Lily's house. So, the dare was to go to the mirror and say "Bloody Mary" three times. Mary was supposed to appear and scratch out the eyes of the reciter.

Instead of this happening, Mary appeared to people who'd hidden a secret death in their past. In this case, it was Mr. Shoemaker. His wife had died of a drug overdose, but apparently he was somewhat responsible for that outcome. It did not mean murder, Mary did not require that much to liquefy a person's brains and remove their eyes.
7. "Bloody Mary"- Sam and Dean discovered that Blood Mary was really a woman from Indiana that had been murdered several years ago. What was her name?

Answer: Mary Worthington

Mary Worthington had been murdered by the doctor she was dating at that time. Her brother had sold the mirror that captured her death to an antique store in Toledo, Ohio, so she was "in the area" so to speak. That explained why the Blood Mary curse had started to actually become real.
8. "Bloody Mary"- Did Bloody Mary try to kill Sam and Dean?

Answer: Yes

Sam and Dean had acquired a picture of the actual mirror and the name of the store it was sold to. Sam wanted to use himself to lure her out and destroy the mirror once she appeared inside it. He thought it was worth a shot, but he was not completely sure it would end her. Dean was outside of the store, as the brothers had set off a silent alarm and the policed had arrived to investigate.

While outside, Sam called Mary out and she was killing him. Dean hit the officers, knocking them out cold, and ran back inside. Sam had already smashed Mary Worthington's mirror, but she hadn't stopped attacking.

He smashed several more, but it did not stop her either. Dean came in an freed her from the mirrors and she attacked them both. Dean and Sam were both bleeding from their eyes, when Dean turned a mirror onto Mary.

This reflection talked to Mary, caused her to bleed from the eyes, then sort of just implode.
9. "Bloody Mary"- Just before Mary's victims began to bleed out through their eyes, what image did they see talking back to them?

Answer: their own

They would be staring into the mirror and see Mary's blackened form behind them. She wouldn't do anything to them, as the stared deep into the mirror. But, suddenly their own image would become free from them and start talking. The reflection would become cruel and taunting about the secret death of the person's past, and then their eyes would begin to drip blood.
10. "Bloody Mary"- At the end, Mary crawls out of her mirror. She slowly comes out, slightly jerking, black hair down all over her head. Which horror film did this scene directly copy?

Answer: The Ring

In "The Ring", an evil spirit named Samara would crawl out of a television set, in jerking sketchy movements with a head full of long scraggly black hair falling all about her face and head. This scene was exactly the same. Mary Worthington was on her hands and knees, black hair and deathly white skin a perfect compliment to the young Samara. Samara would give each person in "The Ring" seven days to figure out how to escape death, after watching a particular VHS tape. Sam and Dean were on the floor, both bleeding from the eyes, as she crawled out toward them. Dean grabbed the nearest mirror and reflected Mary her image back to her.

It became independent of her as well, and mocked her killing the people she'd murdered since her own death. And, in the tradition of Medusa, her own image killed her as well.
