Quiz Answer Key and Fun Facts
1. The following are the brand names of four antibiotics. Which one does not belong?

Answer: Amoxil

Cipro (ciprofloxacin), Levaquin (levofloxacin), and Noroxin (norfloxacin) are all members of the fluoroquinolone class of antibiotics. Amoxil is amoxicillin, a member of the penicillin family of beta-lactam antibiotics.
2. Losec (omeprazole), Prevacid (lansoprazole), and Nexium (esomeprazole), are all members of which class of medications?

Answer: proton-pump inhibitors

All of these medications affect the GI system in some way. PPI's directly inhibit the transporter that secretes acid into the stomach. H2RA's indirectly inhibit the secretion of acid. Antacids directly neutralize acid in the stomach, but have no effect on secretion. Pro-kinetic agents (like metoclopramide) increase the tone of the GI system, reducing the rate of reflux.
3. If taken improperly, bisphosphonates like Fosamax (alendronate) and Actonel (risedronate) can severely damage the esophagus.

Answer: True

This is very true. Because of this, bisphosphonates are to be taken first thing in the morning,on an empty stomach, with one or two full glasses of water. After taking this, the patient must remain upright for 30 minutes to prevent reflux of the pill. Then, the patient can eat. The presence of food reduces the effectiveness of the pill.
4. Which of these medications is not chemically related to cholesterol?

Answer: atorvastatin

Ethinyl estradiol is a modified estrogen used in oral contraceptive pills, prednisone is a modified version of cortisol used for many purposes, and medroxyprogesterone is a progesterone substitute used in hormone replacement therapy. Atorvastatin is an HMG-CoA reductase inhibitor, which basically means that it can help lower cholesterol levels, however it is not classified as a steroid.
5. I have just been prescribed Arthrotec (diclofenac / misoprostol). Which of the following is false?

Answer: This is the best medication for me if I am a pregnant woman

Misoprostol can cause uterine contractions in sufficient doses, and therefore is not the best choice in pregnancy. A combination of methotrexate (a cytotoxic folate antagonist) and misoprostol given vaginally can induce abortion.
6. Which of the following medications does not inhibit the action of histamine on a receptor?

Answer: Pantaloc (pantoprazole)

Benadryl is a traditional anti-histamine used to treat seasonal allergies. Gravol is also an antihistamine that is also used for nausea related to motion sickness. Zantac is a histamine-2 receptor blocker that helps reduce the acid levels in the stomach. Pantoloc reduces acid levels as well, but it's a proton-pump inhibitor.
7. Medications such as Elavil (amitriptyline) and Tegretol (carbemazepine) can be used to treat certain types of pain disorders.

Answer: True

These medications can be used to treat certain neuropathic pain disorders like trigeminal neuralgia or pain secondary to nerve injuries (such as after a car accident). Elavil is normally used as a tricyclic antidepressant, while Tegretol sees use in treating epileptic seizures.
8. Which of the following medications has nothing to do with the renin-angiotensin-aldosterone system of blood pressure regulation?

Answer: Adalat (nifedipine)

Renin activates angiotensinogen to angiotensin I. Angiotensin Converting Enzyme (ACE) converts Angiotensin I to Angiotensin II. Altace (ramipril) is an ACE inhibitor. Avapro (irbesartan) is an angiotensin II receptor blocker. Left unchecked, angiotensin II binds to the receptor and prompts the release of aldosterone which promotes the loss of potassium and the uptake of sodium leading to fluid retention. Aldactone (spironolactone) prevents aldosterone from acting.

These three classes of medications reduce the degree of fluid retention, reducing blood pressure. Adalat (nifedipine) is a calcium channel blocker that has more of an effect on arrhythmia and angina than blood pressure.
9. Which of the following statements about asthma medications is true?

Answer: Corticosteroid inhalers like Flovent (fluticasone) are to be used daily

Short acting agents like salbutamol are used to treat an acute attack, while long acting agents prevent attacks. Neither of these "beta-agonists" should be used more than prescribed, or they may cease to have any effect (tachyphylaxis). Spiriva is used to treat Chronic Obstructive Pulmonary Disease (COPD) as well as asthma. Steroid inhalers are the backbone of asthma therapy, and should be used daily to prevent attacks.
10. Which of the following over-the-counter creams is an antifungal?

Answer: Canesten (clotrimazole)

Polysporin cream is an antibacterial, and Cortate cream is a topical anti-inflammatory cream. Clotrimazole creams are used in varying strengths to treat athlete's foot, diaper dermatitis, and vaginal candidiasis (aka yeast infections).
