1. Seven of Nine's human last name.

Answer: Hansen

Annika Hansen was born to Human parents who decided to research the Borg. Unfortunately, they got too close and dropped their guard, and were assimilated. Annika Hansen became Seven of Nine, Tertiary Adjunct of Unimatrix 01 and was later rescued by Voyager in "Scorpion."
2. Delta Quadrant species that places great emphasis on "the hunt."

Answer: Hirogen

The Hirogen were first seen in the episode "Hunters" when they captured Tuvok and Seven, who had gone to a relay station in order to get the rest of the message sent to Voyager from Earth. A group of Hirogen took over the ship in "The Killing Game," forcing the crew to live through holodeck simulations of hunts from settings as diverse as World War II and a Klingon world.

In order to free her people, Janeway made a deal with the Hirogen and gave them holodeck technology. In "Flesh and Blood," the characters in the Hirogen hunts killed the Hirogen and fled, led by a Bajoran holodeck character who believed he was a God.
3. Admiral whose encrypted message was finally decoded by an alien who wanted to hand over Voyager's crew to the Borg.

Answer: Hayes

Admiral Hayes' message was decrypted by the alien in "Hope and Fear" to say that Starfleet had sent a vessel (the Dauntless) for Voyager to use to return to the Alpha Quadrant. However, after finding out that the ship was of alien technology, the message was (correctly, this time) decoded to say that Hayes regretted that Starfleet had not found a way to bring Voyager home. Now the big question is: Why would a message about failing to help the ship come home be so heavily encrypted?
Janeway said that Admiral Hayes was a bit of a windbag.

Answer: holo-imager

Chakotay pretended he was taking photos while actually trying to find out why (and how) Starfleet Headquarters had been recreated in the Delta Quadrant. Turns out that Species 8472 was preparing to invade Earth.

Mark, her fiance, wrote a letter saying that he had held out hope longer than most others, but decided that he had to move on. Only a short time before he heard the news that Voyager had not been destroyed, he had married.

In "Heroes and Demons", he went into the holodeck to try to find out where crewmembers like Harry Kim had disappeared to. The Doctor ended up fighting Grendel, the monster in Harry's holoprogram of "Beowulf."

Voyager's crew, stripped of all technology, was forced to try and make fire for heat and find food. Unfortunately, one crewmember was killed by a giant lizard in a cave. By saving one of the indigenous people, Chakotay was able to earn their trust and they even helped save little Naomi Wildman's life with their (albeit crude) medicine.

The Holy Pilgrim, as the story went, was supposed to lead the Great Sages (played by two Ferengi) back to the sky in a ball of fire. Unfortunately, the townspeople believed the fire bit and almost burned Neelix and the Ferengi at the stake.

Answer: Hoover

Fred, holding a phaser, asked Voyager to put him into contact with J. Edgar Hoover. Unfortunately for Fred, Hoover was long dead and couldn't help him.
