This is revealed to you partially when you call EVA after fighting Ocelot and then at the end when the boss reveals that her child was taken away from her after she gave birth on the battlefield. The player can imply that Ocelot is her son.

2. What kind of injuries does Snake sustain when you check the survival viewer after Snake is on the riverbed at the end of the virtuous mission?

Answer: A fractured left elbow and rib bone, lacerations to his upper arms, right elbow and abdomen

A paramedic tells you via codec when you fall off the bridge at Tselinoyarsk. Snake gets the call while he is laying motionless washed up on the ground.

3. What did Snake not pick up on about the picture of Volgin and Raikov that EVA gave him?

Tatyana, Cynthia, Tanya and of course EVA are the three names. EVA is how she is known to Snake, Tatyana is how she is known to Volgin, Cynthia is revealed to Snake at the cave in Tikhogornyj and Tanya is how she is known to Granin.

10. According to Snake, why doesn't the Patriot ever run out of ammo?

Answer: Because the internal feed mechanism is shaped like an infinity symbol

In order to access this codec conversation you must first equip the Patriot and then call Sigint. After Sigint asks Snake about why it doesn't run out of ammo, Snake explains it to him and suprisingly, Sigint agrees that that will give you infinite ammo.
