1. How did Beleg Strongbow die?

Answer: Slain by Túrin Turambar

Beleg Cúthalion (the Strongbow), the Great Archer of Doriath, participated in the Hunt for Carcharoth, the Great Wolf of Angband, with Beren. He wielded the bow Belthronding and the blade Anglachel, which was made from meteoric iron. Later he became best friend to Túrin Turambar and became his companion. Beleg went to Túrin's rescue when he was captured and he hewed Túrin's chains with his sword.

But Turin, blinded by the enemy, only saw a vague figure above him, holding a sword. Thinking this figure (Beleg) was about to strike him, Túrin wrestled the blade Anglachel from Beleg and slew him. Only then Túrin realised it was his friend who was with him. Beleg was buried with Belthronding, his mighty bow of black yew-wood.

But Túrin took the sword Anglachel, so it could take revenge on Morgoth.

It was reforged and named Gurthang. There is a distinct similarity between the character Beleg Strongbow and Bard the Bowman (in "The Hobbit"). Both were great archers with a great bow out of black yew wood and both had a charmed black arrow. Beleg's charmed arrow, Dailir, was broken when he carried Túrin to a safe place after rescuing him (and just before he was slain by him), a sign of doom.
2. How came Mablung of the Heavy-Hand to his end?

Answer: Slain by Dwarves

Mablung was the Chief-Captain of Doriath. He participated in the hunt for the wolf Carcharoth and cut the Silmaril from the wolf's belly. He was a friend of Túrin Turambar and was one of the last who saw him alive. He was killed in Menegroth by the Dwarves when they attacked and sacked Doriath.
3. How did Ecthelion of Gondolin die?

Answer: Killed by Gothmog, Lord of the Balrogs

Ecthelion was one of the Captains of Gondolin. When Maeglin betrayed the fair city of Gondolin it was assaulted by Morgoth. To Gondolin he dispatched one of his most powerfull Lieutenants, Gothmog, the Lord of the Balrogs. He duelled with Ecthelion, Elf-Lord of Gondolin (the Stewards of Gondor were possibly named after this Ecthelion). Both slew each other in this battle.
4. Glorfindel of Gondolin was a mighty Elf-Lord. How did he die?

Answer: Slain by an unnamed Balrog

Glorfindel remains a highly debated character. The major issue is whether Glorfindel of Rivendell is a reincarnation of Glorfindel of Gondolin or whether this Elf was named after him. Tolkien was never clear about this and there are hints in both directions. Glorfindel of Gondolin was a mighty Elf-Lord and one of the Captains of Gondolin.

He survived the sack of the city and fled with the remaining survivors. At Cirith Thoronath the survivors were ambushed and one of the Balrogs had pursued them to avenge its Lord. Glorfindel and the Balrog battled and both fell into the abyss, plunging to their deaths. Thorondor, the mighty Eagle flew into the chasm and retrieved the body of Glorfindel afterwards.
5. Gil-galad was High King of the Noldor when they resided in Middle-Earth during the Second Era. How did he die?

Answer: Killed by Sauron

Ereinion Gil-galad was the 6th and last High King of the Noldor and the son of Fingon (Ereinion was his true name, meaning 'Descendant of Kings'. He was commonly known as Gil-galad, 'Star of Radiance'). He formed The Last Alliance of Men and Elves with Elendil the Tall, survivor of Numenor. Together they marched against Sauron at the end of the Second Age.

They defeated Sauron at the Battle of Dagorlad and laid siege to the Barad-dûr for seven years in which many fell, including Anárion, son of Elendil.

At last, Sauron himself came out and he was overthrown by both Gil-galad and Elendil, who perished themselves. Gil-galad's Spear, Aeglos (the Icicle) broke in his fall.
6. How did Isildur die?

Answer: Killed by Orcs after an ambush at the Gladden Fields

Isildur cut the Ring from Sauron's hand. Instead of destroying it, he kept it. Two years after the defeat of Sauron a party of Orcs waylaid Isildur and his small company. All were slain, save for three including his esquire, Othar. But Isildur tried to escape swimming over the Anduin, using the ring.

The Ring slipped from his finger and he was shot by the Orcs. His three elder sons were slain as well. Othar carried the shards of Narsil to Imladris (Rivendell).
7. Celeborn was the spouse of Galadriel, one of the Noldor. Galadriel took the last ship into the West with the last ringbearers and the last remaining Noldor in III, 3021. This included Elrond, Gandalf, Bilbo and Frodo. Did Celeborn go with her on that ship?

Answer: No

Celeborn was an Elf of Doriath and married the Lady Galadriel with whom he had one daughter, Celebrían, the wife of Elrond. In the War of the Ring he was leading the armies of Lórien against Sauron's forces in the North and was victorious and Galadriel destroyed Dol Guldur.

After the War, the last of the Noldor (including Elrond and Galadriel) moved to the West. But Celeborn, being a Sindar, remained in Middle-Earth. He had dwelt in Lórien for a while and then went to Rivendell to dwell with the Sons of Elrond for a time.

But his ultimate fate is unknown. It is known that by IV, 120 he had left Lórien. In that year Elessar died and Arwen went to Lórien. It is mentioned in the Appendix that Celeborn had left Lórien and no Elves dwelled there anymore.

He fades from memory at some point and no one knows what happened to him. Some guess he might have sailed into the West (see the Prologue of "The Lord of the Rings", last paragraph) to return to Galadriel but this is by no means sure. Also he was one of the Sindar, those of the Teleri that never made it to Aman. On the one hand we have his wife being in Aman, but on the other hand, his kind never went there. We can't be sure what his final fate was but he certainly didn't go to Aman on the ship that Galadriel took.
8. How did Maedhros, son of Fëanor, die?

Answer: Flung himself into a chasm

Maedhros and his brother Maglor were compelled by their oath to steal the Silmarils from the victorious Valar armies after the War of Wrath. They slew the guards who were guarding the Jewels and took them. But by their evil deeds they had lost the right to carry them and the Jewels of their father were burning in their hands. Maedhros couldn't endure the pain but refused to separate from it and he flung himself into a fiery chasm.
9. Who was the Lord of Dale, who was killed in the assault of Sauron on Erebor and Dale?

Answer: Brand

Brand, son of Bain, son of Bard, was the Lord of Dale during the War of the Rings. He fell with Dáin Ironfoot, King under the Mountain, trying to defend Dale against the Easterlings. Their people took refuge in Erebor and after Sauron's defeat they came out and broke the Siege. Bard II, son of Brand became the new Lord of Dale and Thorin III Stonehelm succeeded his father Dáin. Bard the Bowman was the courageous archer, who slew Smaug over Lake-Town roughly 70 to 80 years earlier. Girion was the Lord of Dale when Smaug destroyed it in times long ago. Bard was his heir.
10. Who killed Azog before the Gates of Moria?

Answer: Dáin Ironfoot

Thrór was killed by Azog when he returned to Moria. His son, Thráin, rallied great numbers of Dwarves to avenge his death and one by one all Orc strongholds were destroyed. Before Moria a final large battle raged. Thorin was driven back with Thráin, but Náin of the Ironhills drove against Azog. Azog broke Náin's neck and was slain in turn by Náin's son, Dáin Ironfoot.
11. Who killed Bolg, son of Azog?

Answer: Beorn

Bolg was the son of Azog, whom Dáin had slain in Moria. The Orcs charged against the Elves of Mirkwood, the Men from the Lake and the Dwarves of the Ironhills as they were gathered in front of Erebor, negotiating with Thorin, King under the Mountain. Thorin came out and drove against Bolg but was mortally wounded.

In the last hour of the Battle of the Five Armies, Beorn appeared and carried Thorin away. He returned, scattered Bolg's guard and crushed the Orc leader.
12. Where did Thráin, son of Thrór and father of Thorin II Oakenshield, die?

Answer: Dol Guldur

Thráin was captured by Sauron in Dol Guldur and left to die in the dungeons. Gandalf could not save him and the last of the Dwarven Rings was lost.

Answer: Her fate is unknown

Her final fate is unknown. Sam had injured her severely with Sting and used the Phial to injure her eyes. Whether she died or slowly healed her wounds is never told but it is doubtful that she died when Sauron was destroyed since she had been in Middle-Earth long before him and did not belong to him.

Helm Hammerhand was the last King of his line. He was found frozen to death. The valley before the Hornburg was called Helm's Deep after this.

Aragorn Elessar, King of Gondor, died of old age in year 1541 of the Shire reckoning (IV, 120). Arwen left Gondor after his death to dwell alone in Lórien for a while before she lay down and died as well.

Thrór was killed as he tried to enter Moria. The Orc Chief, Azog, had carved his name into Thrór's head and threw it outside with some pieces of gold. Nár had waited outside and was told to leave and bring news to the Dwarves that Moria had a new master. Nár left but over his shoulder he saw how the Orcs chopped Thrór's corpse to pieces.

He brought the news to Thráin, Thrór's son. Upon this, the Dwarves gathered in huge numbers and started smoking out all Orc Strongholds until they came to Moria. Náin was slain in the Battle before the Gates of Moria and Thráin would perish many years later in the dungeons of Dol Guldur.
17. How did Boromir II, son of Denethor II, die, during the War of the Ring?

Answer: Killed by Uruk-Hai

Boromir tried to take the Ring from Frodo. Frodo used the Ring and fled. Shortly after that, the Uruk-Hai attacked. Boromir fell trying to defend Merry and Pippin.
18. Who killed Túrin Turambar?

Answer: He killed himself

Túrin slew Glaurung, the father of all Dragons. But evil sorcery had made him wed his own sister Nienor Níniel. When she found out that Túrin was actually her brother she killed herself. Túrin killed himself by casting himself on Gurthang, his sword, which broke asunder.
19. And finally, the two Dark Lords. First Morgoth: what was his final fate?

Answer: Cast into the Timeless Void

Melkor was one of the mightiest among the Valar. He became the first Dark Lord, Morgoth, and rampaged through Middle-Earth. He was brought to Valinor and to live among the Valar, but with the help of Ungoliant, the Giant Spider, he defiled the land and destroyed the Two Trees.

He fled back to Middle-Earth. At the end of the War of Wrath, in which the armies of Valinor utterly routed Morgoth's forces, he fled into the very depths of his stronghold and begged for peace. The Valar showed no mercy and hewed the feet under him and chained him.

He was thrown through the Door of Night beyond the Walls of the World into the Timeless Void, where a guard is placed for all time.
20. How was the power of the second Dark Lord, Sauron, finally broken?

Answer: Broken when the One Ring fell into Mount Doom and was unmade

Sauron the Maia was Chief Lieutenant to Morgoth, the first Dark Lord. He not only survived the fall of Morgoth but managed to evade the grasp of the Valar. He continued the work of his master and made war upon the Elves until he was eventually captured by the Men of Númenor. Sauron was taken to Númenor and corrupted the Men of Westernesse and convinced them to assail Valinor.

The Valar called upon Ilúvatar, who destroyed Númenor. Sauron's body was destroyed when the Island Númenor was swallowed by the Sea but he returned to Middle-Earth and took shape again.

He was overthrown by Gil-galad and Elendil and his body was again destroyed when Isildur cut the Ring from his finger. His spirit survived and started to take shape again and at the end of the Third Age he unleashed his full might.

The One Ring had been carried to Mordor unnoticed, though, and Frodo claimed the Ring as his own. Gollum, who had kept the ring for many years wrestled with Frodo and bit the finger with the Ring off. In joy he danced and fell into the Mount Doom.

The Ring was destroyed and Sauron's power came to an end. Being a Maia his spirit was immortal (like Gandalf and all Ainur) but his power was broken forever and Middle-Earth freed from his reign.
