Freja Ollegard, a Danish volleyball player, appears on "Simply Heidi" after her breast cancer awareness website was trolled by "Skankhunt42", which is actually Gerald Broflovski trolling from his home while drinking a glass of red wine. In the second episode of South Park's twentieth season, Ollegard says that trolls will not intimidate her, which simply encourages Gerald to continue doing so. "The Damned" opens with the show "Simply Heidi" having Ollegard as a guest, talking about trolling.

The host, Heidi, tells Ollegard that she has been the victim of trolls, and Ollegard responds that trying to ignore them has not worked.

She calls the trolling disgusting but says she still believes in the goodness of humanity. The officers show him, to his relief, a picture of Eric Cartman, and say that he has quit Twitter. The officers explain that this is different from other people quitting Twitter, because when people quit social networks, they usually "leave at least three paragraphs stating that they are quitting Twitter before they quit Twitter." The officers also tell Gerald that they are trying to determine whether this was a Twitter Suicide or a Twitter Homicide. When Gerald asks his son, Kyle, if he has seen Cartman, Kyle gets scared and goes back to his room. He then calls his friends, as they all played a role in getting Cartman to quit Twitter in the previous episode.

When Garrison is asked by Holt how he would deal with ground troops in Syria, he completely ignores the question and confesses he doesn't know what he's doing, and had no idea he would get this far. Garrison continues saying he should not be president because he will ruin country beyond repair, and that Clinton should be elected because she at least has some experience and "not as bad as you think". Clinton's advisers smile, but immediately, much to their disbelief, Clinton responds "my opponent is a liar and he cannot be trusted. What he is saying is simply not true. Do not believe it." in a rather robotic voice. This leads one of her advisers to declare: "oh my god, she's such a turd sandwich."

This episode, and the entire season, bring back a joke from season 8, episode 8, where the choices for the school mascot are a "Giant Douche" and a "Turd Sandwich", in the most literal of ways, as you would expect from "South Park". The episode aired before the 2004 presidential elections, sending the message that often, Americans are left with two bad choices and are forced to pick one when it comes to politics. This was especially relevant in the 2016 presidential election, given that both Clinton and Trump were the most disliked candidates in history, according to an ABC News/Washington Post poll conducted in late 2016.

His name is a reference to tokenism, as he is the only African-American child in the Colorado town other than his girlfriend at one point, Nichole Daniels. Cartman wears a "Token's Life Matters" t-shirt in reference to the Black Lives Matter movement, which was born in 2013 in the United States to protest issues like racism and police brutality.

He continues to say many other outrageous things, but his supporters keep clapping and cheering for him.

Garrison's desperation seems to hit a nerve with Randy, since he suddenly starts to identify with Garrison.

In the following scene, Randy visits Stephen and tells him that he changed his mind about Garrison's candidacy. Stotch, though, has also changed his mind and now says he cannot vote for that "douche." Randy wonders what is going on, and why people are changing their mind suddenly about everything. Stotch says that "nobody knows what to think anymore", and then offers Randy 'member berry pie. As Randy looks at the pie, he notices the berries (introduced in the first episode of the season as talking berries that utter nostalgic and sometimes racist comments about missing the old times.) When Randy notices what the berries are doing, he decides to head to Hillvale Farm to investigate the member berries.

Meanwhile, on the other side of the world, the Danes are mourning the death of Freja Ollegard. The speaker announces that Denmark has a plan to destroy the troll and "get the troll to come out of hiding."

After reading the note and looking around the parking lot, Gerald goes into his car and desperately clears evidence of his trolling, including his browsing history and emails. He then destroys his phone with a hammer, and, as if this wasn't enough, he frantically drives home and takes his desktop computer apart, removing the hard drive and tossing it into a body of water. When Gerald goes back to his home, he realizes his son Ike has another computer, so he races to Ike's room. In the computer, he finds an email and then a Google Alert that says "I know who you are, meet me under the freemont bridge, 9am tomorrow."
