This episode began with a flashback of Jack, where he and his father went over some x-rays to examine a patient who wants surgery but has a spinal tumor. The patient's name was Angelo and Jack's father told Angelo how he could not be a candidate for surgery.

He advised Angelo how he needed a miracle instead of surgery but Angelo actually told Jack's father how he came for Jack's medical opinion, not his father's. Angelo was portrayed by Ronald Guttman, who originated the role of Alexander Cambias, Sr. on the ABC soap opera, "All My Children", in 2003.
