The legendary West Indian all-rounder Sir Garfield Sobers was the first cricketer to achieve this remarkable feat. On 31 August 1968, while playing for Nottinghamshire against Glamorgan in a County Championship match.

As the link above shows, at this time it has not been done in a Test match. Three players have managed it in an international T20 match, and one in an ODI (international 50-over a side) match - although this was a player for the USA against Papua New Guinea, so hardly the elite of the sport!
