In the beginning of this final installment, Pere Callahan, Jake, and Oy enter the Dixie Pig in 1999 New York City to find the evils that were awaiting Susanna-Mio's arrival. Although they have the power of the Sköldpadda (Can-Tah to "Desperation" readers), Callahan is unable to fight off the vampires that he once destroyed and he kills himself with Jake's Ruger as he and Oy escape through the kitchen. Meanwhile, Susannah and Mia have given birth to the son of Roland and the Crimson King, Mordred, and he kills Mia in order to grow. Susannah takes the opportunity to fight back and she kills everybody in the room (including Sayre). When she shoots at Mordred, she accidentally misses and clips off a single leg of the spider-like creature. She also shoots out Nigel the robot's eyes fearing that he may be siding with the vampires and low men, but it turns out he wasn't. Regardless, Nigel carries Mia to the final door linking Fedic to 1999 New York where she waits for the remaining members of the ka-tet.

This event is in the start of the novel after Susannah-Mio has left the Calla through the door in Doorway Cave using Black Thirteen and it interupts the ceremony held by cheering citizens of the Calla, who have been saved by the 'Ka-Tet of Nineteen'. With the breakers working away at the beams from Thunderclap, one has snapped free, but the path of the beam that the ka-tet is on (Shardik to Maturin) remains intact.

They head to the Manhattan Restaurant of the Mind and discover that after Jake left the store, Enrico Balazar's goons pulled up to speak with Calvin Tower about the vacant lot containing the all-important rose.

To do this he has to talk to Odetta Holmes' parents to assure them that she is not dead, and that they need the money for the company. After their meeting, Roland and Eddie head to 19 Turtleback Lane (Cara Laughs) and they head through what seems like another doorway.

After a quick palaver, a man named Donald Callahan (a familiar face to Stephen King fans) approaches them and asks for their help. He also mentions that he has the Black Thirteen in his church in Calla Bryn Sturgis.
