From Quiz Truck Movies

Answer: It's a legendary hallucination that Swayze's character saw .

Several cultures have had folklore tales of evil black dogs in one form or another. The legend was brought to contemporary times in the movie when the black dog visions appeared to truckers when they got greedy and pushed themselves too hard. In "Black Dog" Swayze had a Black Dog vision when he wrecked and accidentally killed a motorist on the side of the road. The accident sent him to prison. Upon his release he agreed to haul a load to pay off his back debts. Meat Loaf played the owner of the truck. Before long Swayze realized he'd been conned into transporting illegal arms and ended up in a web of intrigue between the people who hired him and the FBI. Tennessee, New Jersey, Connecticut and areas of Route 66 are among the states that supposedly have roaming black dogs as apparitions in one form or another.

Fess Parker (Disney's Davy Crockett and TV's Daniel Boone) played "Doc" Grayson who was called on to save the son of a Sioux chief. In the film Stallone was scheduled to arm-wrestle the actual reigning champ, but it was decided that it may not have been authentic if he beat him. Rick Zumwalt portrayed a champ in the movie named "Bull" Hurley whom Stallone beat in the finals.
