Rick, Shane, Lam, and Leon shot two of the criminals, not knowing there was a third. That third one shot Rick and put him in a coma.

2. Where did Rick believe Shane was while laying on the hospital bed?

Answer: Bathroom

Shane delivered Rick flowers and Rick got up weeks later, believing Shane just delivered the flowers recently. He realized he was gone awhile when he noticed the flowers were dead.

Rick was sitting out front of his house after searching for Lori and Carl. After Duane hit him, he believed Duane was Carl.
