Yes, it's an urban myth.
Werner Heisenberg -- "He lies here, somewhere."
This is a joke about the famous Heisenberg Uncertainty Principle, which implies that one may not know the position and momentum of a particle simultaneously.

star's answer seems to say that it is NOT an urban myth.
However the photo of his grave from the findagrave reference does not show any such inscription.
