1. Both the book and movie started off near the end of the story. Which one gave a flashback?

Answer: both

Most of the movie and the book was just a big flashback from the narrator. Those who believe his name is Tyler Durden are wrong, the narrator's name is never given.
2. In both the book and movie Tyler goes to save Marla from her accidental attempted suicide. In which did he notice a certain item in Marla's apartment that she told him wasn't a threat to him?

Answer: both

I can't really say what that item is, but if you've seen the movie or read the book you'll know what it is. In the movie Tyler had found the phone off the hook from when Marla had called the Narrator. In the book Marla called back and Tyler picked up.
3. In both the book and the movie Tyler meets the Narrator after the Narrator falls asleep. In which did they meet on an airplane?

Answer: movie

In the book they met at an empty nude beach when Tyler has finished making a natural clock that was only perfect for one minute. Tyler then says that a minute is all that you should expect of perfection.

In the book the chapter ended with "I pulled the trigger" and then continued with him in an asylum that he referrs to as Heaven (he calls his doctor God). In the movie the Narrator got up, bleeding crazily, and asked the Project Mayhem members to leave him alone with Marla, who in the movie isn't so willing to be around the Narrator.

The Narrator fell asleep at work, so Tyler took over and filled his boss' computer with gasoline. When the Narrator goes to work a few days later the office has been blown up and his boss is dead.

In the movie he had a bruised and swollen face a lot of the time, but in the book it was a lot more. He had a hole in his face, half his tongue chopped off by his own teeth, and even a broken nose. The Narrator was better off in the movie.

In the book it was a member of Project Mayhem rather than Tyler Durden. The man had been given the job of making the Narrator "let go" (as Tyler put it in the movie) by Tyler.

In the book it hinted at the fact that civilization would still be destroyed, but it hadn't actually taken place in the movie. In the book the building that was to be blown up was just being used as a tool to smash a museum, rather than being a target itself like in the movie.

In the movie the Narrator had disabled the bomb, which led to one of the strangest fight scenes in cinematic history. In the book, however, Tyler had made a mistake while creating the explosives and they didn't work.
10. In both the book and the movie Tyler and the Narrator are on a building that is meant to be blown up. In which does the Narrator shoot himself, thus killing Tyler?

In the movie it went on for a little bit afterwards with him talking to Project Mayhem members and Marla. In the book it automatically cut to the next chapter which took place in the asylum the Narrator had been put in. There he was told that everyone is a "beautiful, unique snowflake", which is a coincidence considering the thing all Project Mayhem members told each other was "You are not a beautiful unique snowflake!"
