From 'Growing Pains'. Mike Seaver gave him the nickname while they were waiting for their first bus ride to school. More trivia: Boner's father's name was Sylvester. Yes, Sylvester Stabone. 'Who knew?' shrugged Boner.
2. Dr. Douglas Howser was better known as:

Answer: Doogie

From, of course, 'Doogie Howser, M.D.' Could you go to a doctor named 'Doogie'?
3. Arthur Fonzarelli went by what nickname?

Answer: The Fonz

From 'Happy Days'. Trivia: Contributors to the National Zoo in Washington DC are known as 'Friends of the National Zoo', or FONZes.
4. Fugitive James Adams was the title character of his series. But of course, they used his nickname, which came from his association with his companion. This nickname was:

From 'The Life and Times of Grizzly Adams'. Actually, I don't recall anyone in the show addressing him as Grizzly, just Adams.

And as the usual business irony goes, this show was cancelled after just one season, even though two of its young stars were nominated for Best Young Actor awards (Jonathan Ward won his) by the Young Artist Awards, as was the show itself for Most Promising New Fall Television Series.

I guess is was something akin to 'mommy', something an adult normally wouldn't call his mother. Ishka was initially portrayed by SCTV alumnus Andrea Martin, and by Cecily Adams in the following appearances of 'Moogie'.

From 'Star Trek'. Bones is a shortening of 'sawbones', a slang expression for surgeon. Funny aside: in episode 'Catspaw', Kirk, McCoy, and Spock were chained up in a medieval dungeon. McCoy was staring anxiously at the skeletal remains of a previous prisoner. Kirk turns to McCoy and begins to call him Bones, sees his friend staring at the skeleton, then calls him 'Doc'. :)
