In the first book, "Twilight", Edward told Bella some of Carlisle's history. Carlisle was changed by a vampire that stayed in London's sewers. He was repulsed by what he had become and discovered that he was able to survive on the blood of animals. Knowing he had lots of free time, he studied several courses, including medicine. He was very compassionate and wanted to help people and not be a monster.

However, he was very lonely. While he was studying in Italy, he came across the Volturi and was pleased to find more civilized vampires. The Volturi kept insisting he change and drink human blood, but Carlisle only wanted to feed on animals. He left peacefully on his own and hoped to come across others that shared his lifestyle.

In "Eclipse", we learned that Rosalie was engaged to Royce King II, the most eligible bachelor in town. She thought he truly loved her, but he was only interested in her beauty. He tired of killing them, so he left. He found Alice and they joined Carlisle's family.
