1. Where was Voyager when it was transported to the Delta Quadrant?

Answer: The Badlands

Voyager was searching for a Maquis ship that's last known whereabouts were in the Badlands. Both Voyager and the Maquis ship were transported to the Delta Quadrant by the Caretaker. The Maquis ship would eventually become part of the Voyager crew and its Captain, Chakotay, would become Captain Janeway's first officer.
2. Kes, one of the characters on 'ST: Voyager', is Ocampan. How long does her race usually live for?

Answer: Nine years

Contrary to most of the other 'Star Trek' universe aliens, Ocampans only live for nine years; however, they have amazing mental abilities and mature much faster than humans.
3. There was an episode in which Chakotay and Captain Janeway were intentionally left behind on an Earth-like planet because they had contracted a virus with no known cure. What was the name of this episode?

Answer: Resolutions

I made the other three choices up, however they sort of pertained to the themes of the episode. It was the hope of many fans who supported the pairing of Janeway and Chakotay that this episode would spark further romantic 'encounters' in future episodes, however they were to be disappointed.
4. When Voyager first encountered the Borg, who played the Queen?

Answer: Susanna Thompson

Unlike in the 'TNG' movie, it was Susanna Thompson who played the Borg Queen for all the 'Voyager' Borg episodes except the finale, where Alice Krige reprized her role as Thompson was busy filming 'Once and Again'. Majel Barrett played the computer voice and Nancy Hower played Ensign Wildeman.
5. What is the first episode where we see the Q character on 'Voyager'?

Answer: Death Wish

'The Q and the Grey' was next followed by 'Q2'. 'Q Who' was a 'TNG' episode that I threw in there because Q only made three appearances on 'Voyager' sadly... Brilliant character.
6. Who composed the 'Star Trek: Voyager' intro theme music?

Answer: Jerry Goldsmith

Alexander Courage collaborated with Jerry Goldsmith to write the 'TNG' opening theme, Dennis McCarthy wrote the 'Deep Space 9' opening theme and Russell Watson sang the 'Enterprise' opening theme song.

Answer: Mademoiselle de Neuf

Her aliases name is odd, because Mademoiselle de Neuf means Miss of Nine in English. A more direct translation would be Sept de Neuf or Mademoiselle Sept de Neuf.

Answer: Hunters

The other three answers were all episodes from the same season (4), just not the right ones.
9. Which Verdi aria from the opera 'Rigoletto' did The Doctor parody during one of his day dreams in 'Tinker Tenor Doctor Spy'?

Answer: La donna è mobile

He parodied 'La donna è mobile' when, in his daydream, Tuvok became struck by the Pon Farr. He had to ad lib his way through calming Tuvok down and then knocking him out with a sedative. Quite hilarious.
10. In the last episode, who helped Voyager by providing the crew with information about transphasic torpedoes and such, with the intent to bring the ship home?

Answer: Future Kathryn Janeway

While Admiral Paris and Barclay did help with the Pathfinder project, it was the future Kathryn Janeway who provided the intel on the Borg, and told the crew how to adapt Voyager to combat the Borg in a better manner. (She helped with shield modifications, extra hull plating and the fitting of Voyager with special torpedoes.

The designs were brought with her from the future and they made Voyager more than a match for the Borg, who, compared to the re-fitted Voyager, were 30 years behind the time.)
