According to McMurphy, the girl said she was 18, but actually she was 15. McMurphy also said that he thought she looked like a 35-year-old woman. McMurphy was also in prison for five counts of assault. He responds to that accusation by saying, "Rocky Marciano's got 40 and he's a millionaire."

Throughout the movie, the inmates play cards, and eventually they lose all their cigarettes to McMurphy, and then the cigarettes are confiscated by Nurse Ratched, who bans card-playing. Cheswick repeatedly cries, "Where are my cigarettes?" Nurse Ratched tries to calmly explain to him that he gambled away his cigarettes to McMurphy and that he can't win them back because card-playing is now banned, but that doesn't make him happy.

Then, a big fight breaks out. McMurphy breaks the glass window that separates the nurses' office from where the inmates are allowed and he gets a cigarette box for Cheswick, only for Cheswick to complain that it is empty.

The Chief pretends to be deaf and mute, but when McMurphy offers him gum, he says "thank you" and reveals that, despite what people told McMurphy about the Chief earlier in the movie, the Chief can indeed hear and talk. The Chief and McMurphy become best friends for the rest of the movie.

The Chief is almost as tall as the distance between the ground and the basket, and with much effort, McMurphy teaches him how to put the ball in the basket. Eventually, when the inmates play against the staff, every time McMurphy gets the ball, he throws it to the Chief and he puts it in the basket. The Chief also prevents the staff from scoring by blocking the ball when it's in the basket, and knocking it back up so it doesn't go all the way through. Apparently, goal-tending isn't illegal in Asylum Basketball.

McMurphy attempts to teach the other inmates how to fish. However, they all lose interest in fishing when they see him talking with Candy. To make matters worse, Cheswick, whom McMurphy put in charge of steering the boat, leaves his post to watch McMurphy and Candy, leaving no one to steer the boat for a while.

The reason that they don't is never revealed in the movie. He's especially upset that a young 21-year-old man is voluntarily at the institution and not outside enjoying his youth.

McMurphy sneaks in two women and some alcohol and bribes one of the guards to allow this and the inmates throw a party. After the party, Nurse Ratched discovers Billy naked and asleep with Candy. Billy's stutter returns as he pleads with Nurse Ratched not to tell his mom. Afraid that Nurse Ratched will not comply with his plea, Billy commits suicide.

After Billy commits suicide due to Nurse Ratched threatening to tell his mom that he slept with a hooker in the institution, McMurphy attempts to strangle Nurse Ratched to death. The guards save her and and he gets a lobotomy. When the Chief discovers that his best friend has been given a lobotomy, the Chief smothers McMurphy to death with a pillow, believing that he'd be better off dead than living with a lobotomy.

When McMurphy learns that his actions caused him to be sentenced to more time in the institution, he decides to try to escape. He attempts to lift a hydrotherapy fountain, planning to throw it through the window and escape. When he fails to do so, he attempts to convince the Chief to do it.

At first the Chief does not do it. However, by the end of the movie, the Chief and McMurphy have become good friends. When McMurphy dies, the Chief, inspired by McMurphy's confidence and wanting to do what McMurphy would have wanted them to do, successfully lifts the fountain, throws it through the window, and escapes.
