Answer: A player entering the game intentionally plays the puck or makes physical contact with an opposing player before the player leaving the game has left the ice

Rule 2.5 Change of Players. A player entering the game intentionally plays the puck or makes physical contact with an opposing player before the player leaving the game has left the ice. A Bench Minor penalty shall be assessed to the player committing the infraction. Rule 2.5 (e) Note 1.
A player entering the game is accidentally hit by the puck before the player leaving the game leaves the ice. No penalty.
A player with one or both skates still on the bench intentionally plays the puck. Assess a Minor penalty for "ineligible player" or "delay of game". This is because that player is not on the ice and therefore not part of the game.
The goaltender is being pulled and the extra attacker steps onto the ice before the goaltender is within 10ft. of the bench. No penalty. Call a premature substitution. Rule 2.5 (b) Note 3.

Rule 10.2 (e) Face-offs. Situation 16. This is straight out of the rule book. The second penalty acts as a coincidental penalty, and since the stoppage play occurred in the defend zone, that is where the face-off will take place. Even if the attacking team were to take another penalty, even an infinite number of penalties at this stoppage, the face-off would remain inside the end zone.

This is because you cannot give territorial advantage to a penalized team.
