Answer: It is a dead ball, the batter gets 1st, the man on 1st gets 2nd and the man on 3rd stays there

Rule 5.09(f) If a fair ball touches an umpire working in the infield after it has bounded past, or over, the pitcher, it is a dead ball.
The batter gets first and all runners forced to advance get one base - Rule 7.04(b).

Answer: true

Depending on how far out into the outfield the fielder goes (this is strictly an umpire's judgment) an umpire may still call infield fly if the fielder is standing on the outfield grass.

Answer: No

The only time an umpire can overrule another is, if the umpire who originally made the call asks for help, or the home plate umpire interprets the rule differently.

Answer: it is a live ball and the play continues

Since the batter did not throw the bat at the ball on purpose, the play is live. If he threw the bat at the ball intentionally then the batter shall be called out.

Rule 5.09(a)(8) of the OBR states: "A batter is out when: After hitting or bunting a fair ball, his bat hits the ball a second time. The ball is dead and no runners may advance. If the batter-runner drops his bat and the ball rolls against the bat in fair territory and, in the umpire's judgement, there was no intention to interfere with the course of the ball, the ball is alive and in play."

As soon as the ball is touched, the runner is allowed to tag up. It does no matter when the ball is actually caught; all the fielder has to do is touch it.

When the ball hits the batter and an attempt is made to swing at it, it becomes strike three and the batter is out.

Answer: yes

Even though the ball is in his glove touching the bag with his bare hand, is just like touching the bag with his foot. This only works with tagging the base if you tried to tag a runner with your bare hand the runner would still be safe.
