1. What is the name of the main city in Act IV?

Answer: Pandemonium Fortress

The Pandemonium Fortress is a hybrid of two worlds, the clean, bright top half with checkered floors and sparkling white walls and the dark, dank bottom half leading to the first explorable area. Regardless of the atmosphere and world around, the Pandemonium Fortress will always be laid out the exact same with its stairs leading south, deeper into the chasms of Hell and the wastelands preceding them.
The Pandemonium Fortress is one of only three Waypoints found in the fourth act of the game and like the previous towns of the game, it contains your trader, gambler, and Deckard Cain.
2. Are any quests completable in the Outer Steppes?

Answer: No

The Outer Steppes is the first explorable area found outside winding staircase in the south end of Pandemonium Fortress and it should give you a clear taste of what's to be expected for the rest of the Act. The area is filled to the brim with undead creatures, Tortured Souls, and others.

Although the enemies in here are comparatively weaker than those later on in Act IV, they are still a jump from the difficulty of monsters in the forests of Act III. The Outer Steppes does not contain a waypoint and it connects directly into the Plains of Despair.
3. What is the name of the monster which must be defeated in the first quest of Act IV?

Answer: Izual

Izual is the focal point of the very first quest of Act IV, 'The Fallen Angel'. To advance in this quest, you need to make your way through the Outer Steppes entirely and find the Plains of Despair. Once here, finding Izual is the entire task, but when he's approached you'll notice that he's one of the stronger bosses of the game. With a Cold Enchantment, Izual will prove to be a formidable foe, even though he's on his own for the most part.
Once 'The Fallen Angel' has been completed, you can return to the Pandemonium Fortress in order to claim your reward from Tyrael: two skill points.
4. Which area connects the Plains of Despair and the River of Flame?

Answer: City of the Damned

The City of the Damned, like the Outer Steppes, has no discernible reason for existence in the game's questline, so your time there will inevitably be used to find the staircase leading deep into the River of Flame. Excluding this, many of the enemies found here will be identical to ones already seen and the atmosphere will not have changed since the first two areas visited in Act IV.

Many of the enemies found here will not carry over into the remaining areas of the Act.
5. What is the name of the second quest in Act IV?

Answer: Hell's Forge

Following 'The Fallen Angel', you'll need to turn your sights directly to the proceeding quest, 'Hell's Forge', which takes you to the River of Flame. The only way to complete this task is by taking a second fork in the road on the River of Flame before you reach the parallel pathways that come after you run into Hadriel.

Hephasto the Armorer must be defeated in order to beat this quest, and for easier access, a waypoint can be found in the River of Flame near Hadriel. It is the final waypoint of the Act (the final of three). You must speak to Deckard Cain to receive this second quest.
6. What must be destroyed in the River of Flame?

Answer: Mephisto's Soulstone

As part of the Act's second quest, 'Hell's Forge', you need to bring Mephisto's Soulstone, received after defeating Mephisto in the final quest of Act III in Kurast's Durance of Hate, to the Hellforge to be destroyed. After you defeat Hephasto the Armorer (similar to The Smith in the penultimate quest of Act I) this quest can be completed. You need to smash the Soulstone on the forge itself with the Hellforge Hammer (dropped from the boss) and as a result the item will split into numerous Flawless and Perfect Gemstones of all types.

After this, speaking to Tyrael will result in news about your final task.
7. Which of these enemies can not be fought in the River of Flame?

Answer: Oblivion Knight

The River of Flame, up until its point in the game, contains one of the highest concentrations of difficult enemies. Blood Maggots, for example, are similar to the maggots found in the Far Oasis of Act II in that they will create small egg sacs of additional, smaller enemies. Urdars are bulky, large enemies which will use their brute force to induce knockback in enemies. Stranglers are large wraith-like enemies which will zap opponents to cause damage. Oblivion Knights, the odd ones out in this question, aren't fought until the final area of Act IV, appearing directly after the completion of the River of Flame.
8. What is the final area found in Act IV where the final quest can be completed?

Answer: Chaos Sanctuary

Home to the strongest creatures in Act IV and one of the more difficult bosses of the entire game, Chaos Sanctuary rests in a north-east direction after you complete the River of Flame. The large pentagram seal in the center of the final chamber is the one from which the Prime Evil, Diablo, will spawn when you aim to complete the final quest of Act IV.

The area is haunted by Oblivion Knights and Doom Knights as well as the magical Storm Casters and the heavy, winged Venom Lords. Before the addition of the "Lord of Destruction" expansion, Chaos Sanctuary was a haven for item drops and runs.
9. How many seals do you need to open in Diablo's chamber?

Answer: Five

This directly pertains the the third and final quest of Act III, 'Terror's End', in which you need to defeat Diablo himself. To complete the quest, you need to open five seals surrounding the large, central seal in Chaos Sanctuary. Three of these seals, when opened, will spawn unique boss enemies (Infector of Souls, Lord de Seis, and the Grand Vizier of Chaos) and two will not, but all seals must be opened before the game's namesake will emerge.

This boss battle is particularly difficult because you will be at the mercy of the beast's fire damage and freezing spells as well. Furthermore, the monster's brute strength will probably cause a good deal of grief.

When the demon is defeated, his drops will be quite substantial as he is the end-of-Act boss.
10. Who must you speak to after defeating Diablo in order to proceed to Act V?

Answer: Tyrael

For those of you that do not have the "Lord of Destruction" expansion for "Diablo II", this is the end of the road. The journey ends when you speak to Tyrael regardless and you will have completed your difficulty level and earned a title for your character ('Slayer' for Normal difficulty, 'Champion' for Nightmare' and 'Patriarch' or 'Matriarch' for Hell if you play Normal mode as opposed to Hardcore). For those with the "Lord of Destruction" expansion, the game is not over. An additional six quests await in Act V of the game, set in Harrogath. To go to this final city, you need to speak to Tyrael and travel via a portal.
