1. The game begins with Detective Castellanos and his team heading to which of these destinations?

Answer: Hospital

Castellanos and his team-- Kidman and Joseph-- head to Beacon Mental Hospital to report for a call only to arrive and find that the place, in lockdown, was the site of a horrible massacre. Investigating for only a few minutes, all of them are thrust into a nightmarish world that, as the player finds, begins in the hospital's depths.

Much of the first chapter involves hiding and running from a disfigured man wielding a chainsaw, and an escape is made very early on, but the only way to get to the bottom of it all is to backtrack and, somehow navigate a way back to Beacon.
2. The gel that allows you to upgrade your weapons and abilities is which colour?

Answer: Green

Green Gel comes in a handful of forms throughout the game. While you're likely to find Green Gel in tidy, glass jars, the substance is also dropped by enemies upon being killed or being burned and destroyed. Green Gel is used when Castellanos goes into the alternative hospital realm.

In addition to saving in this relatively safe space he can, after the first chapter, gain access to a room with an upgrade chair. Each upgrade costs Green Gel and can affect weapons, ammunition, health, strength, or carrying capacity depending.
3. Which of these allows you to access the Safe Haven?

Answer: Mirrors

The Safe Haven is the mysterious and closed-off site where you can save your game, upgrade Detective Castellanos' equipment and body, and do a number of other helpful things to ensure you survive your trip through the game. To get to the Safe Haven, you can locate rooms marked with bloody doors, often accompanied by the faint sound of classical music.

In these rooms, there is always a mirror which can shatter and grant you a passage to where you need to go. The Safe Haven is also known as the Asylum and it's operated by nurse Tatiana Guiterrez, who went missing after working at Beacon.
4. To burn bodies, Detective Castellanos carries which of these?

Answer: Matches

One of the handful of consumable items you can find throughout the game is Matches, and while you can only carry a few from the outset, upgrading them via the Safe Haven provides a big advantage as some foes can be set ablaze before you draw attention to yourself. Matches are also useful, especially later in the game, for opening up passageways that would otherwise be inaccessible due to...otherworldly means.
5. On most occasions, you'll be able to locate safe keys within which of these?

Answer: Statuettes

The Safe Haven, in addition to being a place to save the game or upgrade, also acts as the home to a room lined with safes on its walls. Each of these can be opened, and if they are they'll reveal a prize of some sort inside. Often, these are Green Gel (and often, those are of copious amounts) but on occasion they will also contain ammunition and, interestingly, other keys. Once the item inside has been claimed the safe will shut, but any door will stay open until that point, so you can leave and come back when you need to pick something up for replenishment. Keys are, more often than not, found when you break open statuettes of goddesses, found throughout the game.

This can be done with melee attacks or, if out of reach, bullets.
6. How many different types of bolts can be made for the Agony Crossbow (excluding any DLC/pre-order additions)?

Answer: Five

The Agony Crossbow, first able to be found in Chapter Three, is a versatile weapon that has the ability not only to fire unique ammunition, but to have its ammunition made on the fly from the inventory screen using parts salvaged from disabled traps. In addition to a basic harpoon shot, the Agony Crossbow also fires Explosives, Flash shots and Freezing and Shocking bolts. Each of these acts in separate pools for ammunition purposes, but you need to swap between them to use them.

The "Fighting Chance" DLC pack (and game, if pre-ordered) also adds Poison bolts and Incendiary shots for the weapon.

While the Crossbow itself can be upgraded, so too can each individual ammo type.
7. Which of these is used to replenish Detective Castellanos' health?

Answer: Syringe

Although special kits are also found throughout the game to increase your health bar at different points, health is either built up via the Safe Haven chair or replenished using Syringes found throughout the game. These, like weapons, can be added to your quickbar for easy access though, like weapons, there is a cooldown time required while you make use of the item. Syringes replenish a set amount of health upon use, though this amount can also be increased via upgrades.
8. There are twenty-eight of which of these items found throughout the game?

Answer: Map Fragments

Nearly every chapter of the game contains a small amount of Map Fragments-- large, square tiles which immediately head to the Safe Haven when picked up. When fully collected, they form a large map of Krimson City on the wall in a room next to the safe room and, if viewed, their completed form will unlock two strong weapons which can not be obtained any other way-- a Burst Handgun and a High-Penetration Sniper Rifle.

There are twenty-eight in the game with some chapters having up to four.
9. The final boss battle of the game is held against which of these people?

Answer: Ruvik

Ultimately, the game will lead you to a final battle against Ruvik in the top tower of the Beacon Mental Hospital where the STEM machine rests-- at least in Ruvik's mind-- and it will trigger a battle sequence across a nightmarish dreamscape. The battle is fought in phases, some of which simply involve running through destroyed structures and some of which have you impaled on a piece of rebar. Ultimately, it comes down to destroying Ruvik's brain, the cause of everything, to free yourself from the STEM machine you've incidentally been hooked up to this entire time.
10. Which of these is not a DLC pack for "The Evil Within"?

Answer: The Trial

While "The Evil Within" itself is a fully-contained game, players who purchased the Season Pass or individual DLC packs following its release would have come across three continuations, the first two of which ("The Assignment" and "The Consequence") featured Juli Kidman as a playable character and the third of which ("The Executioner") put the player in the role of The Keeper-- one of the creatures that reappears during the game, fighting with a locked safe on its head. Pre-orders also released with "The Fighting Chance Pack", granting a few in-game bonuses for health, upgrades, weapons, and ammo.
