The CIA sends in three operatives who land while the Bay of Pigs invasion is actively taking place. It is meant to be operating in alliance with, but independent of, the US army.

Answer: Assassinate Fidel Castro

The entire basis for Mason, Woods and Bowman being here in the first place is eliminating Castro. It starts with the team dressed in civilian clothing, at a bar, meeting with an informant. The situation escalates when suspicious police approach them and things immediately turn to bloodshed, kicking off the mission.

Answer: M16

As the scene at the bar quickly escalates to gunfire, Bowman tosses Mason an M16 before the player is allowed to start playing as him. The M16 comes with an underbarelled grenade launcher and an ACOG scope.

For a sidearm, the player receives an ASP pistol.

Answer: Skorpion

There are exactly three enemies inside Castro's manor who wield Skorpions. Many, many enemies will wield the FN FAL, RPK and KS-23. Exactly one enemy has an RPG, though there is no way to get more ammo for it, so the player can't use it for very long. Finally, it is possible for the player to pick up M16s with random attachments off of fallen AI-controlled US army soldiers.

Answer: Taking down the target with a headshot

When the player kicks down the door to Castro's bedroom where he is hiding, the gameplay slows to a cinematic crawl. The player is allowed to aim their gun and fire slowly here. Castro has hid himself behind a human meat shield. The player must kill him without killing the hostage; if they shoot the hostage, they fail and must try again.

Generally, it is actually far easier to kill Castro specifically with a headshot than anywhere else, as his head is the only part not directly being covered by the hostage the player isn't allowed to shoot. As such, this achievement is basically earned by default; you would have to go out of your way not to earn it on accident.

Answer: M60

This M60 is a mounted turret rather than an LMG. It does not have a clip size and wields infinite ammunition, but it is limited by an overheat system. If the player fires too much for too long, it will overheat and have to cooldown. Sadly, it does not come with a reticle, so the player has to make do with terrible iron sights always seen on M60s across "Call of Duty".

Answer: Dead Ops Arcade

Roughly about a third of Dead Ops takes place in Santa Maria, Cuba in the very same area Operation 40 occurred in. The main difference is that Dead Ops is a top-down experience played, fittingly, like an arcade game, whereas Operation 40 is from the first person.

Answer: Castro's Bedroom

Right after killing Castro, there is a dresser directly to the left of where the player broke down the door. Intel is right on top of this dresser. Interestingly, players who have no-clipped into this area and accessed it before the slow motion cinematic have found that the intel does not spawn into the game until after Castro has been killed.

Answer: No

It turns out they actually took out a very convincing double of Fidel Castro, not the main man himself. This is discovered right at the very end during a cutscene.

Answer: He is captured

The team boards Carlos' plane to escape. As they begin to take off, they spot a stacked row of cars that would be sure to deny their escape. In an act of sacrifice, Mason opts to jump off the plane, get onto a turret, blow up the stack of cars and allow the plane to properly take off without him. He is then promptly neutralized and captured by the Russians.
