Answer: Hades "Hercules" 1997

Hades is immortal, so technically he can't die, and he didn't.
2. Which villain, even though he was defeated, still HANGS around?

Answer: Clayton "Tarzan" 1999

Clayton died by accidentally hanging himself, so he still HANGS around.

Answer: Madame Mim "The Sword in the Stone" 1963

Madame Mim did indeed turn into a dragon, but the last time we see her she is in her human form again, albeit sick with a thermometer in her mouth.

Most female villains use magic. Yzma turned herself into a cat accidentally, but Madame Mim has green eyes, and therefore, fits all of the above.
