Here' the list: "Time and Again" (Kes says the Capt. died here), "Deadlock", "Coda" (4 times for this one), "Before & After" (refered to as dead), "Worst Case Scenario" (holographic Janeway dies), "Year of Hell", "Living Witness" (Doc: "Somewhere, halfway across the galaxy, I hope, Capt. Janeway is spinning in her grave!), "Timeless", "Course: Oblivion" (duplicate dies), "Barge of the Dead" (B'Elanna's dream), "Tinker Tenor Doctor Spy" (in Doc's fantasy), "Shattered" (referred to as dead), "Relativity" (Janeway is on the bridge when the ship explodes in an alternate timeline.), "Endgame"

One Janeway died when Voyager self-destructed in "Deadlock," Admiral Janeway was killed by the Borg Queen in "Endgame," and in a temporal anamoly that Kes experiences Janeway and Torres died during a Krenim attack in "Before & After."

B'Elanna initiated the core breach in "Dreadnought." Janeway flew through the binary pulsar in "Scientific Method," rammed her ship into the timeship in "Year of Hell," and insisted on being assimilated by the Borg in "Unimatrix Zero" and "Endgame."

In "Author, Author," the character of Capt. Jenkins is based on Capt. Janeway in the Doctor's holonovel. Jenkins is also played by my Mulgrew, and she shoots a crewmember so that the Doctor can treat Lt. Marseilles. Lt. Marseilles' character is based on Lt. Paris.
