Answer: Shaak Ti

Minutes before the Great Jedi Purge, Ti encountered Anakin Skywalker, whom she believed was on his way to the Chancellor's office to help Mace Windu, Agen Kolar, Kit Fisto and Saesee Tiin arrest the Chancellor. She tried to stop him from leaving, but failed. Had she succeeded Anakin may never have become Darth Vader.

I survived the Great Jedi Purge and I was responsible for Darth Vader wearing his black armour. I was finally killed by Darth Vader on the first Death Star.

Answer: Obi-Wan Kenobi

That was an easy one! We all know Obi-Wan saw Darth Maul kill Qui-Gon Jinn on Naboo, and then dealt with the Sith Lord himself. He later defeated Darth Vader, but left him alive. Another easy one! Yoda fled to Dagobah to escape the Great Jedi Purge. He stayed there for the rest of his life. He trained Luke Skywalker in the ways of the Jedi. Luke left before he finished his training and when he returned he found Yoda in poor health. Yoda confirmed to Luke that Darth Vader was Luke's father and with his dying breath told Luke there was another Skywalker.

He then died, aged 900 and faded as he became one with the Force.

The other Jedi were quickly killed by Palpatine. Mace Windu seemed to have the upper hand when Anakin Skywalker arrived. Palpatine convinced Anakin that he had the knowledge to save Padme from dying. Anakin severed Windu's hands, leaving him defenceless against the full power of Palpatine's Force lightning. Palpatine hurtled him out of the window into the city below.

Answer: Ki-Adi-Mundi

Ki-Adi-Mundi fell to Order 66 on Mygeeto. He led the clones into battle. Suddenly they stopped firing and he turned around, confused, to see them pointing their blasters at him. He managed to deflect a few bolts but was soon overwhelmed. When the Galactic Empire emerged official records stated that Ki-Adi-Mundi was executed because he was arming an explosive device on one of Mygeeto's bridges.
15. I was a Nautolan Jedi Master. I fought in the Battle of Geonosis, where I disabled a battle droid accidently built with the head of C-3PO. I died at the hands of Chancellor Palpatine whilst trying to arrest him.

Answer: Kit Fisto

Kit had a short chat with Agen Kolar and Saesee Tinn before their gunship left to take them to the Chancellor's office. He was wrong.
16. I was an Iktotchi Jedi Master. I never trained a Padawan of my own. I was one of the few Jedi promoted to Master without doing so. I was a very skilled pilot.

Answer: Roan Shryne

Roan Shryne fought on Murkhana during the execution of Order 66. He, Olee Starstone and Bol Chatak were spared when Captain Climber deliberately misdirected an ambush. Answer: Anakin Skywalker

Anakin Skywalker turned to the Dark Side to gain the knowledge to save his wife from dying. Ironically, he was responsible for her death. She lost the will to live when she saw what Anakin had become. Anakin was left very burnt and disfigured after his confrontation with Obi-Wan Kenobi. This led to him having to wear his black armour and breather mask. The rest is history.

Along with fellow Jedi Djinn Altis and Geith I remained in contact with Master Plett who was sheltering a group of Jedi children on Belsavis. The Empire sent a massive dreadnaught, the Eye of Palpatine, to attack Belsavis. Geith and I tried to destroy the ship. Geith was killed and I ended up a disembodied ghost haunting the dreadnaught's gunnery computer.

Answer: Callista Ming

30 years later Jedi Master Luke Skywalker and others found the Eye of Palpatine and helped Callista destroy it. Callista and Luke fell in love and just before the dreadnaught's destruction Callista transferred her spirit into the body of Cray Mingla.
