A hungry man hightails it to the coast in an old truck. Reaching the northern coast of Prince Edward Island, he steals a fishing boat and rides out, bound for any escape from those who are surely following behind him. Arriving on what he finds to be Falstaff Island, a remote location a few kilometers offshore, he follows a small pinprick of light coming from a solitary shack where Scoutmaster Tim and five boys on an expedition are spending the night.

Tim, also a doctor, offers the mystery man, clearly ill, a place to rest. During this time, the man destroys the radio, knowing people are after him, before he throws up on Tim; spreading his unknown infection. The boys, meanwhile, are locked in their room for the night.
2. The first day after the infection's arrival, Tim tells the boys to go on a hike. Why do they do this?

Answer: To earn a merit badge

The next morning, the five boys are told to go on a trek around the island to earn a merit badge while Tim stays in with the sick man. He doesn't let on that in the night he, too, started experiencing symptoms, becoming weirdly hungry. He gives them the walkie-talkie and sends them off.

The boys take a trail around the island and make a fire, returning after dark to find Tim waiting outside, warning them not to go near the cabin or enter under any circumstances since the man inside must be quarantined.

He doesn't tell them that the man woke up mid-day, somehow changed, and tried to eat the couch. All he can say for sure is that the man is sick...he just doesn't know with what.

Tim, recognizing that he's acting weird, tells the boys to stay outside the cabin but manages to convince himself to bring Max, a boy who happens to be the coroner's son, inside to help. At the same time, he takes their single cooler of food outside. Tim, however, does not inform the boys that he's lost twenty pounds over the course of the day.
Using a scalpel and a soldering iron, Tim opens the now-emaciated man on the couch, noting that he looks inhuman because of the disease. When he does, he and Max are shocked to find a white, worm-like creature inside the man's stomach and after hacking at it with a scalpel, Tim accidentally kills the patient. The worm, however, continues to move and, at one point, stretches more than three feet in length. Fortunately, it doesn't touch either of them.
Outside, Kent decides that as the leader of the boys and the son of the local sheriff he'll need to check in on the situation.
4. Kent is the first boy to get infected, but from what?

Answer: A drink of scotch

The boys, unsure of what to do about Tim, lock him into a closet with a combination lock and Kent, feeling proud, takes a swig from Tim's flask of scotch, still on the counter. This proves fatal in the long run; the infection has already been transferred to the flask via Tim's saliva. The boys don't know the island has been quarantined during the day.
While the five boys question if hiding Tim away was right, they also worry that the boat may not come for them the next morning.
In the night, a voice in Tim's head tells him to eat, though there's nothing in the closet to satisfy his growing hunger. Meanwhile, someone tapes the door shut, blocking any light from coming in.
The next morning when the boys wake up, the cooler of food is missing. Kent looks horrible.

The boys find the cooler a short way down the beach-- empty-- and soon they realize that no boat will be coming for them. A storm begins to pick up, so the group of them make their way back to the shelter of the cabin but, afraid of the dead man inside, they opt to hide out in the cellar. This is when Shelley tells the others the truth: Kent took the cooler and ate the contents. He's quite sick with what Tim has.
Max heads into the cabin to ask Tim for help and when he peels the tape off the bottom of the door he finds blood. Tim, inside, placidly tells Max that they were right to lock him away, and they need to find shelter.
Meanwhile, Ephraim beats up Kent when the former strongest boy tries to bullrush into the cellar; Max and Newton bring him to the woodpile for shelter instead where, waiting scared under a tarp, he eats bugs. Ephraim, is left with Shelley who, in the shelter, claims he sees something move under Ephraim's fingers...sparking worry, of course.
While the boys are safe in the cellar, a tree crashes through the cabin roof. Worriedly, they let Kent in with them, hiding him in a corner. It's only when the storm dies down that they find the cabin has been virtually destroyed. Tim has been crushed by the tree, and when the boys find him, white worms have spilled out of his stomach.
6. What happens to the boat that is taken to Falstaff Island during an unexpected rescue attempt?

Answer: It's blown up

The following morning, a boat heading to the island is intercepted by the military and blown up; the boys see this from a distance. Newton heads to the cabin to collect his field guide and ropes, using his sleeping bag to protect himself from the worms.

This proves to be a smart idea; the worms are capable of shooting silky strands at moving targets, and any single one could spread the infection. Max, Ephraim, and Newton go off to find food elsewhere on the island while Shelley stays back to 'play' with Kent, feeding him insect carcasses and the remnants of the first worm from inside the house.

They boys don't know what Shelley is-- a sociopath, and possibly a murderer. He lied to Ephraim about the worms inside him and, soon, he plans to use the walkie-talkie he slipped into Eef's pocket.
7. Max and Newton kill which creature in their hunt for food, immediately regretting their actions?

Answer: Turtle

Ephraim, worried about the possibility of infection under his skin, waits alone in a copse of spruce trees while Max and Newton continue to look for food. After they depart, Shelley ends up using the walkie, catching Eef at the right moment to urge him to use a swiss army knife to catch the worm inside and pull it out. In his paranoia, Eef starts with his hand.
Max and Newt find blueberries and laxative mushrooms for Kent, thinking that forcing the infection out could be the best option. They build traps, but find a turtle and catch it, bringing it to shore and flipping it. Max stabs at the creature, but after watching it die, the boys decide to bury it without eating it.
Ephraim, meanwhile cuts into the ankle and believes he sees something. Soon after, he looks to his stomach.
Shelley, now bored, decides to kill Kent, bringing him to the water and drowning him for the sake of it. In Kent's struggle to breathe, Shelley accidentally ingests some of the water he sprays back, infecting himself in the process.
8. Is Ephraim actually infected by the parasite at any time?

Answer: No

After killing Kent, Shelley drags him back through the water, but his former troopmate's scalp detaches and white worms rush out, attaching to him, perhaps finding their way inside. A few hours later, on the shore, Shelley began to eat. The next day, the worm wraps its way up his spine in the cabin, attaching to his cortex and taking over.
The boys return to Ephraim and find him severely wounded, having cut himself all over, begging for the worm inside to be removed. The boys try to reassure him that he's healthy, but to no avail. Instead, they offer the laxative mushrooms to him, but only when they return to the cabin where it's safer to take them. He agrees to this.
While Newton and Max head to the beach, Shelley emerges from the cabin and, provoking Ephraim further, cuts into the boy when he's willing. When Shelley gets Eef to admit that he wants to burn the worm out, he decides to use the gasoline from the generator to accomplish the task.

Max and Newt return to the cabin to find Ephraim engulfed in flames. While they manage to put him out, it's too late to do anything. Shelley, now different, rounds the corner of the cabin. While Newt notices the boy is sick, Max doesn't care; he approaches to fight.

When he reaches Shelley, though, he's met with a knife to the abdomen. Newt has to interfere, knocking Shelley out from behind. Max's wound proves shallow, and after burying Ephraim the two resolve to leave the island as quickly as possible.

The next day, they head into the cabin to retrieve their boat's replacement spark plugs, only to find that the mystery man ate them upon his arrival. They retrieve these and leave them by the boat, but Shelley takes them when they search for the gas tank in the cellar. Soon, they're in hot pursuit.
10. Who is the only boy to survive the trip to the island?

Answer: Max

When Max and Newt find Shelley's hiding spot, a dark cave, they have no choice but to go inside...where they find the spark plugs waiting. Shelley springs out from the darkness a changed being; his limbs have extended and his skin is taut. His eyes are virtually missing; only his stomach protrudes, barely hanging on. The boys try to flee without the spark plugs, but Newton caught under the Shelley-thing just as the stomach tears free and bursts open onto him.
The next day it's apparent that Newt is infected. Max returns to the cave alone while Newt sleeps and gets the spark plugs, but not before awakening and alerting the worms inside. Fortunately he's able to escape and both boys are able to leave on the boat.
On the water, the boys aren't surprised to see military boats arrive to meet them, and from there, Newt is shot.
Max survives the ordeal, but is taken to a quarantine clinic for further investigation. When he finally tests negative he's able to go home. He misses the others, of course, but he has the curious urge and hunger to go back to the island.
