Jordyn Wieber (USA) actually qualified 4th out of 24 top qualifiers. However, in artistic gymnastics there's a two per country rule, which states that only two gymnasts from each country can advance to a final. Therefore, if two of your teammates qualify ahead of you then you cannot qualify to the same final no matter how high your score is. Both of Jordyn's teammates, Gabby Douglas and Aly Raisman qualified ahead of her and so Jordyn was left out of the finals.

Aliya Mustafina actually had a tied score with four persons on four different occasions at the 2012 Olympic Games. They are Elizabeth Tweddle on floor in qualifications, Aly Raisman (USA) in the All-Around final, Vanessa Ferrari (Italy) on floor in event finals and Jordyn Wieber (USA) on beam in qualifications.

At the 2012 Olympic Games all ties had rules in which they can/should be broken and only one winner emerges. It is a requirement that all gymnasts land feet first for them to get any score at all. At the 2008 Olympics, gymnasts lost an automatic .8 points off their scores if they fall but still land feet first.

At the 2008 Olympics Hannah Whelan fell on vault and got a score of 13.5 (that means she would've gotten a 14.3 without the fall). Her overall score of 62.232 was the highest all around score recorded at a major international competition for the quadrennium 2009-2012.

Finally, Sui Lu who qualified first on beam was beaten by her teammate Deng Linlin who had .1 difficulty ahead of her and both of them were awarded the same execution score.
