This is one of my favorite scenes in the episode, because Zoe calls Mal and herself "Big damn heroes". When River is about to be burned at the stake, Serenity arrives, opening the hatch Jayne is dangling out of. He is also carrying a cocked rifle with laser sights. Mal and Zoe approach from the ground.

While Mal also needs a doctor aboard his ship, he did not really feel bad for leaving them on Jiangyin as Book was in critical condition and needed a doctor immediately. And while he does not say it to Simon's face, he does appreciate Simon's talents and is glad he has them on board, even though they are "the two most wanted".
